ROLE
The Patient Flow Navigator plays a pivotal role in supporting and optimizing patient flow across the Huron Perth Healthcare Alliance (HPHA). Working in close collaboration with Team Leads, Bed Allocators, Allied Health, Physicians, and the Patient Flow Manager, this role ensures smooth and timely transitions of care while maintaining a patient- and family-centered approach.
The successful candidate will coordinate daily patient flow operations and ensure efficient, high-quality care delivery across all units. In alignment with local and provincial policies and strategies, the Patient Flow Navigator will implement and monitor initiatives to reduce inpatient length of stay, improve access, and enhance inpatient bed utilization across HPHA’s facilities.
Additionally, this role carries accountability for staffing oversight during their shift including:
- Approving short call staffing needs
- Coordinating staffing adjustments
- Reassigning staff as necessary to meet unit acuity and dynamic patient needs
- Planning and maintaining effective 24-hour patient flow coverage
Key Responsibilities:
- Lead and coordinate day-to-day patient flow across all HPHA sites
- Collaborate with interprofessional teams to facilitate timely admissions, discharges, and transfers
- Proactively identify and resolve patient flow barriers
- Reassign staff to meet fluctuating acuity and census across units
- Approve and oversee short call staffing decisions in collaboration with unit leadership and scheduling
- Implement staffing adjustments in real-time to support safe, quality care delivery
- Develop, communicate, and maintain strategic patient flow plans over a 24-hour period
- Ensure alignment with HPHA policies and broader provincial health strategies including the ALC Leading Practices Guide, P4R Initiative, and Ontario Health data findings
- Support discharge planning efforts in collaboration with community partners and internal care teams
- Promote a culture of safety, collaboration, and excellence in care transitions
REQUIRED EDUCATION & CERTIFICATION(S)
- A regulated health professional with current membership in the relevant Ontario regulatory body.
- Current Basic Life Support (BLS) certificate required
- Crucial Conversations or willing to obtain
- Emotional Intelligence course or willing to obtain
REQUIRED SKILLS & EXPERIENCE(S)
- Two years’ experience in care coordination, advocacy and discharge planning in a healthcare setting
- Knowledge of community, government resources and relevant legislation related to patient flow
- Demonstrated leadership and coordination skills, with the ability to oversee and support interdisciplinary teams in dynamic and high-pressure environments
- Excellent assessment, communication, organizational, problem-solving, and decision-making abilities
- Strong emotional intelligence and ability to navigate complex interpersonal and clinical situations
- Proven ability to prioritize and manage multiple competing demands while maintaining a patient-centered focus
- Ability to plan, coordinate, and lead staffing and patient flow strategies to support safe and efficient care
- Ability to work independently as well as part of an inter-professional team
- Demonstrated commitment to and understanding of diversity, equity, and inclusion principles
- Ability to facilitate therapeutic relationships and ensure patient-centered care throughout transitions
DESIRED SKILLS, EXPERIENCE(S) & EDUCATION
- Recent (within last 5 years) experience with patient flow is preferred
- Knowledge of Quality Improvement methodologies such as PDSA cycles is an asset
- 2 years’ recent leadership experience is an asset
