Description:
Your Opportunity

This position is responsible for conducting face to face comprehensive assessments for clients seeking aged care services.

The role is essential in the delivery of personalised, client centered assessments that promotes independence and improve quality of life.

Key Responsibilities

Undertake clinical and non-clinical assessments within the program
Observe clients in their home or community settings to gain insights into their daily routines, environmental risks, and overall needs;
Evaluate medical, physical, psychological, and social factors affecting clients’ eligibility for government-funded services;
Provide clinical expertise in managing complex cases, identifying when additional specialist interventions, re-ablement strategies, or advanced care planning are required;
Actively participate in case conferences with team members to review and address clients' evolving needs; and
Undertake support plan reviews to ensure they remain aligned with the client's evolving care needs, particularly following significant changes in health status, hospitalisations, or new assessments.
Collaborate with clients, their families, and representatives to develop tailored support plans based on individual needs, preferences, and goals;
Ensure the support plan promotes independence, well-being, and improved quality of life;
Use the Integrated Assessment Tool to document and finalise support plans for client approval and submission to relevant aged care services;
Engage with a multi-disciplinary team of nurses, allied health professionals, and medical experts to ensure clinical input where necessary, especially in complex cases; and
Work closely with external service providers to ensure accurate referrals and access to complementary supports such as geriatric, mental health, palliative care, and disability services.

Skills and Experiences:
You will possess the following qualifications and skills: -

Tertiary qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist, phycologists or Social Worker and AHPRA registration (or equivalent registration authority);
In depth knowledge of the Aged Care Sector, including government-funded programs, Aged Care services, and relevant legislation (e.g., Aged Care Act, My Aged Care system);
Proven experience in conducting comprehensive assessments within Aged Care, Disability, Health, or Community Services environments
Sound level of cultural competency, with the ability to provide services to clients from diverse backgrounds
Demonstrated commitment to clinical best practice, ethical service delivery and continuous quality improvement within the scope of the aged care assessment frameworks; and
Current C class Driver’s license (QLD).

Other requirements:
Evidence of an influenza vaccination for the most current influenza season (and ongoing as and when available and directed);
Evidence of a COVID-19 vaccination (two doses) and booster (as and when available and directed);
Working with Children Clearance (Blue Card) and NDIS worker screening (Yellow Card)
National Police Check
