Five Rivers Health Centers is an expanding Federally Qualified Health Center (FQHC) providing a medical home for low-income patients in Dayton and Xenia, Ohio. Position Overview
The Community Health Worker (CHW) must abide by various regulatory compliance rules and must always treat patients with dignity and respect. The CHW will work directly with the patients to help them navigate and access community services while adopting healthy behaviors.
General Responsibilities
- Conducting outreach in the community, with a specific target population, that promotes, maintains, and improves individual and community health
- Provide information on available resources, provide social support, informal counseling, advocate for individuals and community health needs
- Collecting data to help identify community health needs
- Other duties as assigned
- Extensive local travel is required
Qualifications
- High School diploma or equivalent required
- Associates Degree in the healthcare or business field preferred
- 2-3 years of health/social services experience preferred
- Fluency in Spanish and English languages required
- Windows-based computer experience required
- Must have valid driver’s license and dependable transportation
