An excellent opportunity has now arisen for a Care Co-ordinators to join our home care team in Preston, on a permanent full-time basis.
You will be responsible for the co-ordination of our Domiciliary Care Workers and supporting the Registered Manager.
We are looking for an exceptional individual who can deliver a quality service and be proactive within the role.
Role and Responsibilities
- Maintain continuity of care and support to all service users and to ensure efficiency and reliability of all visits to service users by suitably trained and experienced care workers.
- Ensure the effective running of the organisation by providing suitable Health & Social Care Support Workers to Service Users.
- Ensure Holywell Care Services policies and procedures are followed at all times.
- Ensure Care Quality Commission standards are met for service users.
Main Duties and Key Responsibilities
- Process any new business referral's, sending out relevant information in response to privately funded service users enquiries .
- Promote positively the service users right to choice and independence whilst ensuring that the are treated with the upmost respect, privacy and dignity at all times
- Ensure that Care workers receive their weekly rotas in a timely manner, prior to following weeks start date.
- Ensure that all holiday/sickness and emergency calls are documented and reassigned within a timely manner and that all parties are kept informed and regularly updated.
- Implement care plans for service users, and changes to care plans where appropriate.
- Ensure Care plans and Risk assessments are updated on a regular basis in line with internal policies and that updates are recorded on our electronic system.
- Oversee and line manage Supervisions, team leaders and other staff.
- Carry out on call duties, evenings and weekends, on a rota basis to support and assist Support workers, service users, be available to be called out if necessary.
- Attend meetings when required to do so by management.
- Support and facilitate the growth of the business in all aspects of the role.
- Treat all staff equally, fairly and respectfully on all aspects of the role
Job Type: Full-time
Pay: £26,500.00-£29,000.00 per year
