Care Coordinator-Lsw Or Rn

Full Time 40/hour M-F

The Cedars is looking for a full time 40 hour/week Care Coordinator, License Social Worker (LSW) or a Registered Nurse (RN). This position will assist in planning, implementing and Coordinating of services that supports the patient to reach his/her optimal level of physical, mental and psycho-social well being. This position is a liaison between staff, patient and family. The Care Coordinator must work collaboratively with all other health care team members, patients and families to facilitate appropriate discharge plans and quality patient outcomes.

We require excellent communication and organizational skills. Must be a team player and have the ability to work closely with an interdisciplinary team. Must be committed to delivering the best quality customer service.

Requirements:

• Knowledge of discharge planning and care management skills are required.
• Knowledge of Medicare, Maine Care and Managed Care payer sources are desired.
• Knowledge of Medicare and Managed Care Appeal Process
• Must have excellent communication, organizational skills, and possess strong clinical skills.
• Geriatric Care experience is desired.
• RN or LSW

Education:

• Registered Nurse (RN) with geriatric and rehabilitation experience a plus.
• State of Maine Licensed Social Worker
