Kōrero mō te tūranga - About the role

An exciting opportunity has become available for a highly motivated and experienced health professional who has a passion for working with older people, palliative clients and those with Chronic health Conditions. Working within the Community Services Group, Care Managers provide comprehensive assessments for people with complex needs and support the development of a person-centred care plan alongside whanau. Care Managers coordinate, implement and review packages of support to ensure that people’s needs continue to be met.

You will work alongside service users and their whanau to help support and enable the person to self-manage where possible and appropriate. You will work closely with service users, family/whanau, clinicians in the hospital environment and services in the community. You will be responsible for building partnerships and maintaining strong relationships.

This is a person-centred role where care and compassion are needed. You will be organised, efficient and show attention to detail. The role operates across a range of health settings including visiting people in their own homes, hospital, hospice or within residential care settings. Care Managers are based within the Community Service Team at Taranaki Base Hospital and /or Hawera Hospital with an option for some working from home in the role. As you will be required to travel a full NZ Drivers licence is required.

This position has 1.2 FTE available and possibility to job share for the right candidate.

Moū - About you

We welcome all applicants with the following attributes:

  • Hold a professional qualification and current NZ Annual Practising Certificate (or professional equivalent)
  • Have strong cultural awareness and commitment to implement strategies and services that are responsive to Maori and other high need populations
  • Have significant clinical experience and display expert levels of clinical judgement
  • Excellent written and oral communication skills.
  • Be confident in the application of policy and process
  • Have high levels of computer literacy and ideally a knowledge of interRAI assessments
  • Have sound knowledge and experience with assessment and care planning processes
  • Be able to work autonomously and manage client caseload including the associated administrative /reporting procedures.
  • Be creative, solutions focused and work in partnership to problem solve.
  • Have knowledge of a wide range of formal and informal local health and disability support services.
  • Be committed to on-going quality improvement and high levels of service user and partner satisfaction.
