CARE NAVIGATOR – Men's Day Program

Full-Time

Monarch Recovery Services provides programming to men with chemical dependency. We are seeking 1 Full Time Care Navigator to provide connection services to clients who are referred to Monarch Recovery Services, before admission and after completion of programs.

The job duties include following up and maintaining contact with clients who are waiting to enter programs through weekly telephone calls, emails, or in person. Meeting with clients while in the program to help set up aftercare services. To connect on a regular basis with the clients to ensure follow up with aftercare plans, up to 6 months. The navigator will be a member of the Men's Day Treatment team. Facilitation of groups may be a required. The navigator will develop and maintain links with professional colleagues in Sudbury and in other communities to make ensure appropriate resources are available to clients who have completed our programs.

Salary starts at $27.10 per hour

Qualifications:

  • Degree in human services, from an accredited university, with appropriate experience in counselling.
  • Experience and knowledge of addictions, men’s issues.
  • Familiar with Stages of Change and Motivational Interviewing.
  • Good written and oral communication skills.
  • Bilingual in French and English a definite asset
  • Good understanding of community resources
