Job Title: Case Coordinator
Location: Kanjirod, Kannur

Job Summary:
We are seeking a highly organized and detail-oriented Case Coordinator to join our team. The Case Coordinator will be responsible for managing and coordinating the care and services for clients, ensuring efficient service delivery while maintaining a high level of confidentiality. This role involves overseeing case loads, monitoring absences, allocating cases to therapists, and managing scheduling tasks such as filling vacant slots.

Key Responsibilities:


  • Case Load Management:


    • Efficiently manage the distribution of cases among therapists, ensuring an even and manageable workload.
    • Track and monitor the progress of each case, ensuring clients receive the required services promptly.

  • Absence Monitoring:


    • Monitor and manage absences of both clients and therapists, ensuring minimal disruption to services.
    • Coordinate rescheduling of appointments when needed and ensure that clients receive timely care.

  • Case Allocation to Therapists:


    • Allocate new cases to the appropriate therapist based on their expertise and availability.
    • Ensure that therapists are aware of their new cases and any specific client needs.

  • Confidentiality & Record Keeping:


    • Ensure strict adherence to confidentiality protocols regarding client information.
    • Maintain accurate and up-to-date records of client interactions, progress, and services provided.

  • Filling Vacant Slots:


    • Proactively manage the therapy schedule, ensuring that vacant slots are filled in a timely manner.
    • Coordinate with clients and therapists to ensure effective scheduling and avoid service delays.

  • Collaboration & Communication:


    • Work closely with therapists, social workers, and other healthcare professionals to coordinate client care and treatment plans.
    • Facilitate communication between team members to ensure seamless service delivery.

  • Documentation & Reporting:


    • Maintain accurate records of case assignments, session outcomes, and therapist feedback.
    • Prepare periodic reports on caseloads, therapist performance, and scheduling efficiency for management.

Qualifications:


  • Educational Qualification:


    • Master’s Degree in Social Work (MSW) or equivalent.

  • Experience:


Experienced or freshers can apply.
