Description:
The Opportunity

Reporting to the Supportive Housing Team Leader the Case Coordinator is responsible for the assessment, analysis and coordination of services and interventions to people who have experienced persistent homelessness. The Case Coordinator will provide specialist advice to enhance service provision across the team, ensuring individuals are linked to a range of unique support services, networks and self-improvement opportunities that will help them transition to long-term housing and rebuild their lives. In achieving this you will coordinate a range of health, social, creative and skills development programs developed and delivered to assist in solving homelessness and meeting tenants’ personal goals.

Skills and Experiences:
About You

We are seeking someone with sound technical skills and knowledge combined with a collaborative and flexible partnering style, together with a strong commitment to our vision and mission.

  • A degree or qualification in Social Work, Mental Health or Community Services, or other relevant qualification, and/ or equivalent experience in intensive case management.
  • Knowledge of chronic health issues related to people experiencing homelessness including mental health, alcohol and other drug use and comorbidity.
  • Knowledge of the broader human services network in South Australia.
  • Experience in and understanding of outreach service provision in health and community services.
  • Demonstrated empathy and the ability to sensitively build confidential and constructive relationships with a diverse range of people.
  • Ability to work flexibly, creatively and in a solution focused way.
  • Ability to work autonomously as well as cohesively as a part of a team.
  • Well-developed conceptual, analytical, and problem-solving skills with strong decision-making ability.
  • Well-developed organisational skills, including time management skills and the ability to manage conflicting priorities and high work volumes without affecting quality of service delivery.

Please note that it is a minimum requirement of this position that an appointment is made subject to a satisfactory Nationally Coordinated Criminal History Check (Police Check) conducted by Housing Choices Australia and the successful candidate agrees to:

  • undergo any other employment screening as required by Housing Choices - including but not restricted to Working with Children or Vulnerable Persons and NDIS Worker (this will be as applicable based on the state location and/or function of the role);
  • comply with Child Safety policies and procedures; and
  • provide a current driver’s license and willingness to travel across Housing Choices’ locations, if and as required.
  • eligibility for provisional, or full social worker registration
