Case Coordinator

Rehabilitation Support Services, Inc. is hiring a Case Coordinator for its Certified 14 bed Apartment Program. This program serves individuals with mental health diagnoses. Hours and Benefits:

• 40 hour work week, 8am-4:30pm (30 minute unpaid lunch)

Requirements for the Case Coordinator:

• A high school diploma or equivalent
• Valid NYS Driver’s License required
• Successful clearance by the Justice Center and New York State Office of Children and Family Services Statewide Central Register Database Check also required.

Responsibilities for the Case Coordinator include:

• Responsible for a designated case load; including the accurate and timely completion of case record documentation in the Agency electronic health record.
• Establish and maintain good rapport and therapeutic relationship with residents.
• Aid and encourage clients to meet the established goals of their service plans.
• Teach, monitor and evaluate client’s skills of daily living.
• Monitor and Supervise medications with clients.
• Meet with clients at least 5x per month and provide restorative services.
• Assist with transportation as needed.
• Hold a caseload of approximately 7-9 clients.
• Provide compassionate and person centered services.
• Weekly on-call rotation approximately once every three months.
