PG BIG believes in giving holistic, wrap around services to those living with acquired brain injuries & their families.

We provide a comprehensive Case Management Program, engaging & educational groups/classes as well as uphold our commitment to community by ensuring we are promoting injury prevention awareness.

We are looking for a new team member who will be responsible for conducting intakes, setting up and/or acquiring community supports/resources, establishing rehabilitation plans, assisting to meet goals and facilitating various programs within the organization (once trained)

The Case Manager will work within the guidelines, policies, and mission of the agency and reports to the Core Services Supervisor.

Primary responsibilities Provide specialized case management services:

o Brain injury education

o Financial security

o Housing needs

o Coping strategies

o Family support

Provide a central role in ensuring community services/supports are accessed:

o Food security

o Financial needs

o Mental health/addictions

o Medical professionals and treatments

Participate in the on-going facilitation of various groups/programs (once trained).

Qualifications

·Diploma or degree in human service or health

· Valid BC Driver’s License

· Safety oriented first aid & CPR certificate

· Minimum of 3 years of human service experience

· An equivalent combination of education, training, and experience

Job Types: Full-time, Permanent

Pay: From $31.56 per hour

Expected hours: 37.5 per week
