Case Manager - SA/MI:
Consider joining a great team of professionals as part of our growing Substance Abuse Disorders Department for a f.t. position, in a well-established agency in Columbiana County, normally M-F, with comprehensive insurance benefits, generous paid leave, retirement plan with employer match eligibility, EAP, some schedule flexibility, and productivity bonus.
Service provision to clients, families, specific target populations, per assigned agency or other locations. Responsible for providing ongoing services and crisis management to clients with dual diagnosis of substance abuse and mental illness and/or per assigned population. The caseload may include court-ordered individuals or specific referrals. The case manager will provide community services that address the principal needs of the client. The case manager may provide group therapy as assigned. Record documentation, service planning, and progress of client in achieving treatment goals. Participate in individual supervision, group supervision, peer review activities, team meetings, quality assurance meetings. Travel , case consultation, telephone contacts, education and prevention services. Computer data entry. Meet productivity expectations. Timely and accurate completion of documentation and ability to work a flexible schedule based on agency client need, and as determined with supervisor.
REQUIREMENTS:
Education/experience:
Bachelor's degree in field of mental health, social work, counseling, psychology, human/family services), required. with Ohio chemical dependency (CD) certification/licensure in substance abuse preferred; or a (Bachelor's degree in related field such as education, criminal justice, corrections), considered along with minimum of two years' related experience, and same Ohio CD certification/licensure preferred. Minimum of high school diploma or Associate's degree with appropriate Ohio Chemical Dependency certification will also be considered. Applicable experience preferred for all.
Preferred:
Bachelor's degree in related field of mental health, such as social work, counseling, psychology, human/family services; one year paid experience, with service population, and Ohio chemical dependency certification/licensure.
Skills in written and verbal communication, assessment and planning, organization, interpersonal skills and teamwork. Ability to perform computer data entry required; knowledge of Microsoft (Word) preferred. Ability to meet agency required vehicle insurance liability limits, valid driver's license, vehicle in safe operating condition, and continued maintenance of all. Subject to criminal background, BMV, and drug screen clearances. E.O.E.
