CFC Service Coordinator

Program: Child and Family Connections, Early Intervention

Location(s): Arlington Heights (North Suburban Cook County)

Hours: Full time, Monday - Friday, 8am-4pm, 40 hrs/week. Hybrid position with 2-3 days in the office

Pay: $47,750 - $51,750 / year depending on education and experience, plus full -time benefits package

Clearbrook is a non-profit, social services agency that specializes in providing innovative opportunities and services to individuals with intellectual and developmental disabilities.

The Service Coordinator assists and enables a child who is eligible for Early Intervention services and the child's family to receive rights, procedural safeguards, and services that are authorized under the State of Illinois' Early Intervention Program.

The responsibilities of a Service Coordinator include, but are not limited to:

  • Completing Intakes, Coordinating eligibility determination process
  • Assisting families in locating service providers
  • Collaborates with team members to develop individual family service plans (IFSP)
  • Maintaining early intervention records
  • Maintaining up to date accurate information on Cornerstone system
  • Maintaining constant contact with families,
  • Facilitating six-month IFSP reviews, IFSP updates and Annual IFSPs
  • Implementing transition plan
  • Completing the family fee schedule
  • Conducting developmental screenings and public awareness presentations

Minimum Education / Experience Requirements:

  • A bachelors degree or higher in human services, behavioral science, social science or health related field acceptable for the Early Intervention Service Coordinator credential
