Remuneration: $58.95 - $60.86 per hour + superannuation + salary packaging
Hours Per Week: 16 (Job Share may be considered for part time hours)
Location: Mount Druitt, The Hills, Parramatta, Auburn & Blacktown Community Health Centre’s

Where you'll be working

This role sits within Community Health services delivering care to clients with chronic and complex needs across WSLHD.
  • Community-based social work services across multiple sites.
  • Reports to Program Lead and Service Manager
  • Works within multidisciplinary teams including medical and nursing staff
  • Supports service planning, evaluation and quality initiatives
  • Collaborates with clients, carers and external providers

What you'll be doing

You will provide senior-level clinical social work services in complex and sensitive care contexts.
  • Deliver advanced assessment and therapeutic interventions including palliative care
  • Work autonomously with minimal supervision in complex clinical scenarios
  • Support multidisciplinary care planning and coordination
  • Lead service improvement, education and quality initiatives
  • Provide supervision and mentoring to staff and students

Essential Criteria:
  • Qualification: Qualification: Degree in Social Work with eligibility for membership of the Australian Association of Social Work

Selection Criteria:
1. Degree in Social Work with eligibility for membership of the Australian Association of Social Work with demonstrated advanced clinical knowledge, skills and experiences in the provision of community Social Work, including palliative care.
2. Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team.
3. Experience or capability in developing and implementing policies and procedures for service improvement, and in providing training and education with experience in staff and/or student supervision.
4. Well-developed time management skills with excellent written and verbal communication, including report writing skills.
5. Demonstrated experience in quality assurance and / or clinical practice improvement activities.
6. Demonstrated ability to work with consumers, families and carers in a time of high levels of family stress.
7. Good computer skills, including experience using Cerner eMR or equivalent.
8. Drivers license valid within Australia and willingness to use for work purposes.
