Description:
Position: Clinical Care Partner
Location: Belair, South Australia
Team: Help at Home Clinical Team
Direct Reports: Regional Nurses

Position Overview

The Clinical Care Partner plays a crucial leadership role within Kalyra’s Help at Home program, overseeing the clinical care of clients with complex or high-impact health needs. This role focuses on building the capability of the workforce to deliver safe, high-quality, person-centred care that supports reablement, early intervention, and wellness.

Working closely with Registered Nurses, Enrolled Nurses, and Home Support Workers, the Clinical Care Partner provides clinical oversight, guidance, and leadership to ensure service delivery meets both regulatory standards and the unique needs and preferences of each client.

Key Responsibilities

Clinical Leadership & Practice

Provide clinical supervision, support, and mentoring to nursing and care staff
Lead implementation of clinical care strategies and risk management plans
Promote best-practice care aligned with Aged Care Quality Standards and client-centred outcomes
Manage a small caseload of Home Care Package clients

Assessment, Planning & Monitoring

Oversee assessment and clinical care planning for clients with complex or high-prevalence conditions
Collaborate with external health professionals to ensure integrated care and early intervention
Facilitate reassessments and reviews when clients' conditions change

Capability Building & Governance

Identify workforce training needs and facilitate professional development
Lead side-by-side competency assessments and mentoring
Support the implementation of local clinical governance initiatives

Quality & Safety

Investigate and resolve incidents and feedback in line with Kalyra’s quality systems
Ensure compliance with clinical and legislative frameworks including Work Health & Safety
Identify opportunities for continuous improvement across clinical and support services

Organisational Focus Areas

Act as the lead for Assistive Technology and Home Modifications, Restorative Care Pathway, and End of Life Pathway within your region

Salary & Benefits

Hourly rate: from $60.16 per hour (based upon experience)
Vehicle: $10,500 vehicle allowance or a fully maintained vehicle

Skills and Experiences:
What You’ll Bring

Essential

Bachelor’s degree in Nursing
Current AHPRA registration
Demonstrated leadership in clinical practice within aged care or community settings
Experience mentoring or supervising multidisciplinary teams
Sound knowledge of Support at Home, CHSP Guidelines, and the Aged Care Quality Standards
Strong organisational, communication, and problem-solving skills

Desirable

Ability to work across multiple regions
Understanding of relevant legislation, including the Aged Care Act 1997 and WHS regulations
A commitment to person-centred care and continuous improvement

Additional Requirements

Up-to-date vaccinations including influenza
