Clinical Nurse Manager II - Patient Flow Coordinator

PURPOSE OF THE ROLE
The Clinical Nurse Manager II Patient Flow Coordinator will be responsible the management of unscheduled admissions to Tallaght University Hospital. They will have responsibility to provide ready access to beds for patients requiring emergency & elective treatment in collaboration with the Patient Flow Department, Specialist area & Clinical Teams
QUALIFICATIONS
1. Each candidate must on the latest date for receiving completed application forms for the office:
- Be registered in the General Division of the active Register held by Nursing and Midwifery Board of Ireland or be entitled to be so registered.
- Have 5 years relevant post registration experience and a minimum of 2 years’ experience in the acute hospital settings
- Have the clinical, managerial and administrative capacity to properly discharge the functions of the role.
- Have excellent communication and interpersonal skills.
- Demonstrate evidence of continuing learning and professional development
- Be able to demonstrate managerial abilities.
Desirable Criteria
- Evidence of management/ patient flow experience in an acute hospital setting
- Experience working in specialist area
- Knowledge of Hospital patient management IT System
- Have a post graduate qualification not less than level 7 on the NFQ framework
2. Age Restrictions In Relation To Applications
Age restriction shall only apply to a candidate where he/she is not classified as a new entrant (within the meaning of the Public Service Superannuation (Miscellaneous Provisions) Act, 2004). A candidate who is not classified as a new entrant must be under 65 years of age on the first day of the month in which the latest date for receiving completed applications for the office occurs.
3. Health
A candidate for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health as would indicate a reasonable prospect of ability to render regular and efficient service.

5. Garda Vetting
Garda vetting is sought for all employees and prospective employees of Tallaght University Hospital. Given the specialised nature of the services provided, your appointment will be subject to satisfactory Garda Vetting and re-vetting in circumstances where the Hospital deems it appropriate. The Hospital will then process the necessary documentation to endeavour to obtain satisfactory Garda clearance for you. You are obliged to disclose previous and any criminal convictions acquired during the course of your employment. Should the Hospital obtain information from the Garda Vetting Unit to indicate that your Garda clearance report is not satisfactory and / or if you have supplied the hospital with false or misleading information in relation to your Garda clearance status, the Hospital reserves the right to withdraw or terminate this contract in accordance with Tallaght University Hospital Garda Vetting policy.
6. Mandated and Designated Persons under Children First Act 2015Schedule 2 of the Children First Act 2015 specifies the classes of persons defined as Mandated Persons for the purposes of the Act. This includes a range of disciplines that are employed by the hospital including all medical practitioners, registered nurses, physiotherapists, psychologists, social workers and others. This includes staff working in adult services. All mandated persons have two main legal obligations under the Children First Act 2015. Mandated persons, under the legislation are required to report any knowledge, belief or reasonable suspicion that a child has been harmed, is being harmed, or is at risk of being harmed. The Act defines harm as assault, ill-treatment, neglect or sexual abuse, and covers single and multiple instances. Mandated persons are also required to assist Tusla, if requested, in assessing a concern which has been the subject of a mandated report. Designated Officer are required to receive reports of suspected child abuse or neglect from any person who is of the opinion that (a) a child has been or is being assaulted, ill-treated, neglected or sexually abused, or (b) a child’s health, development or welfare has been or is being avoidably impaired or neglected. It is the responsibility of all staff employed by TUH to be aware of their roles and responsibilities under the legislation and to complete mandatory Children First Training.
PARTICULARS OF OFFICE
1. The appointment is full time, permanent and pensionable position and also a panel will be formed to fill any potential future vacancies that may arise.
2. Remuneration:
Remuneration is in accordance with the salary scale approved by the Department of Health & Children. Current remuneration with effect from 1st February 2026:
€62,078 by 11 increments to €78,443 incl. 1 LSI
Salary scales will be updated in line with nationally agreed pay agreements and will be updated accordingly and retrospective payments applied as applicable.
3. Pension Arrangements and Retirement Age
There are three superannuation schemes currently in operation for staff in Tallaght University Hospital:
(a) Voluntary Hospital Superannuation Scheme (Non-New Entrant)
(b) Voluntary Hospital Superannuation Scheme (New Entrant)
(c) Single Public Service Pension Scheme
Please read carefully the summary of the criteria for the three superannuation schemes below. This will assist you in identifying which scheme membership is applicable to your employment circumstances.
(a) If you have been continually employed in the public service prior to the 1st April 2004 and you have not had a break in service of more than 26 weeks, you will be included in the Voluntary Hospital Superannuation Scheme (Non-New Entrant) with a Minimum Retirement Age of 60 and a Maximum Retirement Age of 65.
(b) If you have been continually employed in the public service post the 1st April 2004 and you have not had a break in service of more than 26 weeks, you will be included in the Voluntary Hospital Superannuation Scheme (New Entrant) with a Minimum Retirement Age of 65. There is no Maximum Retirement Age.
(c) If you have commenced employment in the public service as a new entrant or you are a former public servant, returning to the public service after a break of more than 26 weeks, you will be included in the Single Public Service Pension Scheme effective from the 1st January 2013 with a minimum retirement age of 66 (rising to 67 and 68 in line with state pension changes). The maximum retirement age under this scheme will be age 70.
4. Duties:
The CNM2 will perform such duties as are outlined in this job description.
5. Hours of work:
Normal working hours are 37.5 worked over 7 days. Your contracted hours of work are liable to change to meet the requirements for extended day services in accordance with the terms of the Framework Agreement.
6. Probation:
The successful candidate will be appointed initially for a probationary period of 6 months. During the probationary period progress or otherwise will be monitored and at the end of the probationary period the service will (a) be certified as satisfactory and confirmed in writing or if not satisfactory, the probationary period may be extended by 3 months.
7. Annual Leave:
Annual leave entitlement is 25 -28 working days (pro rata), depending on length of service, plus 10 Bank Holidays per annum as they occur. The annual leave year runs from 1st of April to 31st March each year.
8. Sick Leave:
Payment of salary during illness will be in accordance with arrangements as approved from time to time by the Department of Health and Children.
9. Termination of Office:
The employment may be terminated at any time by 2 months’ notice on either side except where circumstances are dictated by the Minimum Notice and Terms of Employment Act 1973/91. The Management’s right under this paragraph shall not be exercised save in circumstances where the Management is of the opinion that the holder of the office has failed to perform satisfactorily the duties of the post or has misconducted himself/herself in relation to the post or is otherwise unfit to hold the appointment.
GENERAL ACCOUNTABILITY
The person chosen will:

- Reporting relationship will be to the CNM 3 Patient Flow/Head of Patient Flow and Access.
- Be professionally accountable to the Director of Nursing.
The overall purpose of the patient flow function is to provide ready access to beds for patients requiring emergency and elective treatment. There are several important elements:
Discharge Planning
- Safe and proactive planning of discharge from day of admission and the management of discharges for patients with more complex interim or long term needs.
- The successful applicant will receive training, to enable them work within the patient flow department.
Bed management
- Safe allocation of beds to patients from various sources on a prioritised basis in accordance with hospital standard operating procedures.
Planned Patient Pathway Boards
- Use of a transparent & electronic visual system of bed management and discharge planning
- The successful applicant will report to the Patient Flow Manager and will receive training, to enable them work within the patient flow department.
SPECIFIC ACCOUNTABILITY
Managing the Service - Quality & Safety of Service / Delivery of Results (level 3)
Bed Management
- Safely allocate beds to patients admitted from all admission sources including ED, Waiting Lists, Outpatients, Day ward and transfers from other hospitals taking into consideration the IPC status of the individual patient.
- Planned Patient Pathway /Visual Boards
- Use of a transparent visual system of bed management and discharge planning
- Review Critical capacity and demand for beds from ED, DOSA & Wards. Prioritise patients based on acuity.
- Be aware of the National Targets set by the HSE, prioritise actions & escalate to the PF CNM 111 & Head of Patient Flow and Access manager if breaching.
- Regularly assess admissions and bed state on a whole hospital basis and prioritise patient allocations based on clinical need.
- Challenge the appropriateness and/or timing of admission decisions when required.
- Interact with the Visual Hospital and Planned Patient Pathway boards.
- Liase with Discharge Planning regularly to identify bed availability.
- Liaise with the CNM’s, Doctors and other members of the discharge planning team highlighting problems and identifying delay factors that require assistance.
- Lead bed management meetings to inform all stakeholders of the bed state and allocation decisions.
- Bed Managers will link daily with external hospitals to expedite patient transfers back to the base hospital where applicable
- Continuously interact with critical care areas within the hospital (ICU, PACU, CCU, HDU and POSU) to determine bed requirements in these areas.
- Ensure patients are in appropriate ward areas according to their specialty and plan for transfer where this was not initially possible.
- Where applicable monitor private bed utilisation and ensure that private beds are fully occupied by private patients whenever possible.
- Interact with waiting list management to ensure HSE standards and waiting list targets are met.
- Escalate any issues that arise that will impede on the patient’s Patient Flow pathway throughout the organisation.
- Liase closely with Patient Flow CNM 3 any relevant issues.
Discharge Planning
- Assist the Discharge Planning Team and Bed Management with the effective and efficient use of hospital beds in accordance with hospital policy. This includes:
- Liaise with the CNM’s, Doctors and other members of the discharge planning team highlighting problems and identifying delay factors that require assistance.
- Perform morning ward rounds to identify definite and query discharges for the day and any predicted discharge for the week.
- Perform weekly ward managements with ward managers and be aware of patients over 100 days.
- Assist in the identification of patients who are fit for discharge and in the co-ordination of a discharge plan.
- Communicate with relevant members of the Multi-Disciplinary Teams on a daily basis to progress with patients discharge.
- Regular communication with Consultants if discharge issues arise.
- Assist in the identification of process flow delays associated with the inpatient episode and the development of more efficient and effective processes.
- Assist in the identification of patients expected date of discharge and monitor accordingly.
- Have some knowledge of long term care, delayed and complex discharges requiring after care. Be aware of the Fair Deal Scheme and other national targets.
- Attend the daily operational bed management meetings, in addition to and in the absence of the Discharge Planner when required.
- Attend the weekend discharge planning meeting when required.
- Liaise daily with offsite rehab/transitional care sites - St Luke’s/Mount Carmel/Peamount ARHC/Resp /NRH/ Clontarf/Interim bed sites – ( Clondalkin Lodge, Care Choice Parnell, Care Choice Malahide, Care Choice Finglas, Care Choice Marlay, Kiltipper Woods )/HSE Egress beds and plan transfers.
- Monitor patients LOS’s regularly and report to patient flow manager any complications early on that may arise to prevent safe discharge.
- Follow up with any delays with the HCP, PHN, equipment, or homeless service either directly or via the MSW.
- Develop, collect and monitor targets/KPI’s which also encompass both national and TUH targets.
- Ongoing review and monitoring of patients who require to be on the Delayed transfer of Care (DTOC) list and escalate as required, ensuring delayed discharges are captured accurately on the HSE Delayed Discharge Portal.
- Prioritise caseload on a daily basis to meet the demands of the hospital & ensure realistic PDD’s are documented & understood by patient’s, family & nursing/medical staff.
- Update HCP tracker and follow up on HCP’s as required.
- Liaise with HCP coordinator /Offsite bed coordinator to plan discharges
- Escalate any blockages early in Patient’s discharge plan.
Patient Placement Co-Ordinator
- Populate the Patient Flow Visual Hospital/Planned Patient pathways boards, working with CNMs and discharge planners to reflect changes in the bed state (discharges, query discharges, admissions/allocations, delays)
- Interact with CNM’s and patient flow staff on expediting discharges. Note all related actions on the Patient Flow /Planned Patient Pathways boards.
- Act as a Patient Flow support, communicating with CNMs to gather information on discharges throughout the day and update the unscheduled care and Patient Flow managers of current status.
- Communicate regularly with Bed Management, Nursing and Discharge Planning staff to assess current bed availability.
- Work with MDT to ensure patients are placed in a hospital bed as soon as possible in order to initiate care.
- Determine if initiating patient care will be delayed and reasons why. Liaise with appropriate staff to prevent delays from occurring.
- Liaise closely with Bed Management to ensure a timely transfer of patient to a bed.
- Assist with setting targets for when beds become available and for patients to be placed.
- Liaise with CNM’s to identify any delays/ bottlenecks in vacating beds.
- Work closely with the Transition Lounge to ensure all patients meeting the criteria are transferred in a timely manner.
- Update Bed Management on available beds and set times for upcoming bed availability.
- Attend Bed Management meetings as requested.
- Awareness of targets set by both Hospital and HSE
- List, monitor and follow up with all diagnostics/reports which may enable same day discharge.
- Continually liaising with ED CNM2 with updates and identifying any delays /roadblocks early.
- Linking in constantly with Bed Managers regarding prioritised beds and transfers and be aware that priorities may change on a day to day basis.
- Have a sound knowledge of the bed management targets and ensure these targets are reviewed daily.
- Attend both pre bed meetings with updates of potential discharges, expected discharge times, discharge lounge suitability, time bed ready. Any issues should be discussed at the pre bed meetings.
- Liaise with the Transition Lounge CNM 11, Ward CNM’s & Bed Managers to optimise early discharge planning, identify discharges the evening before while promoting & optimising TUH “ Home by 11” target.
- Liaise with CNMs with regard to LOS>20 Days and escalate to Discharge Planning Team (DPT) as necessary.
Covering current offsite and future offsite locations: Mt Carmel, Kiltipper Woods, St Luke’s, Care Choice Parnell, Care Choice Malahide, Care Choice Marlay, Care Choice Finglas, Clondalkin Lodge, Tymon, ARHC Peamount, Resp Peamount.
- Clear overview of all TUH patients in off-site facilities from the list above. This list may be subject to change.
- Assist the Discharge Planning Team (DPT) and Bed Management (BM) with the effective and efficient use of offsite beds in accordance with hospital policy. This includes:
- Liaise closely with the DPT, BM, CNM’s, Doctors and other members of the discharge planning team to highlight complex cases and identifying potential delay factors before discharge.
- Contact each off site facility daily for updates on patient status and potential complications to discharge. Site visits recommended on a weekly basis to maintain rapport with staff. Assist in the identification of patients who are fit for discharge to offsite from TUH.
- Attend weekly MDT meetings in offsites.
- Work in partnership with the social work service to ensure effective use of offsite facilities and interim care beds.
- Aim for 100% capacity at all times in all offsite facilities.
- Providing a discharge planning service link between TUH and all offsite facilities for patients & their families who have been identified as having complex needs and who require off site facilities.
- Communicate with relevant members of the Multi-Disciplinary Teams on a daily basis which includes Health & Social Care professionals, nursing and Medical staff to progress with patients discharge.
- Assist in the identification of process flow delays associated with the inpatient episode and the development of more efficient and effective processes.
- Assist in the identification of patients expected date of discharge and monitor accordingly.
- Attend the twice daily huddles in patient flow.
- Promote an awareness of discharge planning in the offsite facility policies throughout the hospital and offsite facilities involved. Ensure compliance from staff on the multi-disciplinary team.
- Ensure that the DPT are aware of when a bed is becoming available & plan the next admission.
Home Care Package Coordination
- Be involved in the management of Home Care Packages for patients of Tallaght University Hospital
- Have a responsibility in managing the home care package tracker and application process with patients and their relevant person.
- Attend regular onsite meetings as required with to assist in the prioritisation of funding for patients that are medically fit for discharge
- Participate in process improvement meetings held between Social Work and Patient Flow.
- Gather relevant data and track delays in the HCP process, be able to provide statistics to hospital management, patient flow and HSE Home Support Office to advocate for additional funding and improved processes.
- Continue to improve working relationships with HSE Home Support Service office staff who have a role in the management and facilitation of HCPs. Regular communication with HCP managers and manager for Services for Older Persons who has oversight of HCPs to ensure timely processing of applications and identifying problems at an earlier stage.
- Internal problem solving and intervention at an earlier stage in the process.
- Early escalation to hospital and HSE management of delays in the HCP process and support around managing complex cases.
- Organise transfer to offsite TUH beds as appropriate while awaiting commencement of package
- Update ward managers, Visual Hospital electronic system and planned patient pathway boards with discharge plan and Predicted date of discharge
- Ensure Bed managers and Patient Placement coordinator are aware of date of discharge.
- Keep up to date with any developments in area of HCP’s both nationally and locally.
Bed Management and Discharge policies
- Participate in the Discharge Planning group.
- Implement and monitor Hospital Bed Management policies and procedures.
- Review the effective functioning of all policies relating to Bed Management /Discharge Planning on a regular basis.
- Promote an awareness of Bed Management and discharge policies throughout the Hospital, ensuring compliance from staff on the multi-disciplinary team.
- Facilitate appropriate transport for patients on discharge, in conjunction with the transport desk, to include ambulance/minibus service within the Dublin Midlands Hospital Group and other HSE regions.
- Implement and monitor processes and polices for TUH regarding the safe transfer of patients to offsite facilities.
- Develop, collect and monitor targets/KPI’s which also encompass both national & TUH targets.
- Review the effective functioning of all policies relating to offsite discharges on a regular basis.
Service Quality
- Support the Bed Manager and clinical teams in placing the right patients in the right place at the right time.
- Be aware of the importance of ring fencing within the Bed Management remit and minimise the short notice of cancellations for elective admissions.
- Participate in Quality Improvements Initiatives/Programmes
- Awareness of targets set by both TUH and the HSE. Maintain a record of clinically relevant data aligned to National KPI’s as directed and advised by TUH & HSE.

Hygiene / Infection prevention and control

- All employees have a responsibility to follow hand hygiene guidelines as this is the single most important intervention to prevent the transmission of infection.
Quality, safety and risk management
- Support the delivery of the Quality, Safety and Risk Management Programme, including the appropriate identification and management of risks and incidents throughout the hospital.
Confidentiality
- You will be aware of the confidential nature of Hospital work and in particular, the right of patients to confidentiality. To this end, you will not discuss workings of the Hospital or its patients or disclose any information of a confidential nature except as required to do so in the course of your work. No records, documents or property of the Hospital may be removed from the premises of the Hospital without prior authorisation. You must return to the Hospital upon request and, in any event, upon the termination of your employment, all documents or other property of the Hospital which are in your possession or under your control.
Data Management
- Ensure compliance with the obligations required by the Data Protection Act 2018.
Development of Hospital Groups
- The Hospital Structure is currently under review and therefore, reporting relationships may change. The development of Hospital Groups may require the post-holder to adopt a different reporting relationship and additional accountabilities. Full consultation will take place in advance of any such change.
NOTE: The extent and speed of change in the delivery of health care is such that adaptability is essential at this level of appointment. The appointee will be required to maintain, enhance and develop their knowledge, skills and aptitudes necessary to respond to this changing environment. He/she will also be required to participate in and support the Hospital’s Digital transformation strategy which may impact work processes and role profiles in the future.
