About the Role
The Bay of Plenty Community Care Coordination Centre serves as a centralised access point for patients needing short-term health services across the region.
The position involves compassionate client interactions and collaborative work with healthcare professionals, using phone and technology-based communications to guide clients toward the best available services.
We are looking for an experienced Coordinator to join us as a Community Care Coordinator, based at Te Whare Whakamana building located on 17th Avenue, across from the Historic Village.
About the Team/Service
Our team of 13 staff are made up of both part-time and full-time, registered and non-registered health professionals, working collaboratively to meet clients’ needs and best possible health outcomes.
About you
As a Community Care Coordinator you will need to have:
- A solution-focused mindset, a collaborative approach
- An eye for identifying opportunities to enhance service integration
- Skills that allow you to screen requests and determining appropriate responses
- The ability to foster key relationships with healthcare providers across the community and hospital settings
