About the Role


Concord Repatriation General Hospital has a well-earned reputation for excellence in healthcare for over 70 years. Building on its proud heritage of caring for the Veteran community, the hospital now services the local communities of Concord, Strathfield, Burwood and beyond.
The SLHD Community Transition Aged Care Program (TACP) serves to transition aged care clients safely from hospital back into the community, while offering opportunities to maximise their function. The TACP data officer collects, collates and reports on the efficacy and quality of the service according to the Australian Government Transition Care guidelines, Ministry of Health, SLHD and Aged Chronic Care & Rehabilitation (ACC&R) policies as required.

Ideal Candidate

  • Demonstrated high level proficiency in the use of PC applications, especially Excel and ability to gain proficiency with new software applications.
  • Current NSW Driver's Licence (P1 and P2 acceptable).
