Responsibilities:
- Using the computer to develop and implement care plans for patients as well as creating goals.
- Speaking with patients POA's, facility staff and patients daily
- Working with the geriatric population
-Medication review with patients. Asking in-depth questions based on their mental health diagnosis.
-Using psychiatric scales for assessments.
- Scheduling unstable visits for the patients as needed
- Monitors adherence to care plans
- Evaluates effectiveness
- Monitors patient progress in a timely manner, and facilitates as needed
- BHI (Behavioral Health Integration)
- This position requires the applicant to visit patients independently in nursing homes as well as work in the office setting while charting
Requirements:
- Strong knowledge of Chronic Care Management/ Behavioral Health Integration
- Excellent communication and interpersonal skills
- Ability to prioritize tasks and manage multiple responsibilities
- Proficient in electronic medical record systems and other relevant software applications
- Strong computer skills
- Excellent communication skills
Salary:
- $16-$18 an hour
- Bonus potential up to $50.00 per assessment
- Mon-Friday 8:30-5pm (with 30 minute lunch break)
- No weekends/No holidays
-Bonus pay available
Job Type: Full-time
Pay: $16.00 - $18.00 per hour
Expected hours: 40 per week
