Position Overview

We are seeking a compassionate and dedicated CNA (Certified Nursing Assistant) or CMA (Certified Medical Assistant) to join our team as a Mental Health Dementia Care Specialist. In this role, you will play a vital part in supporting patients with dementia and other mental health conditions, ensuring they receive quality, individualized care in both facility and office settings.

Responsibilities

• Provide care to patients in skilled nursing and assisted living facilities with a focus on dementia and behavioral health needs.
• Develop and implement patient care plans, including goal setting and progress monitoring.
• Communicate daily with patients, POAs, facility staff, and families to coordinate care.
• Conduct medication reviews and ask in-depth questions related to mental health diagnoses.
• Utilize psychiatric and dementia-related assessment scales.
• Schedule follow-up visits for patients requiring additional support.
• Monitor adherence to care plans and evaluate effectiveness.
• Document patient progress accurately and in a timely manner.
• Support Dementia Care initiatives.
• Visit patients independently in nursing homes and complete charting in the office setting.

Requirements

• Active CNA or CMA certification in Florida (required).
• Experience or strong interest in dementia care and geriatric mental health.
• Knowledge of Chronic Care Management and Behavioral Health Integration is a plus.
• Strong communication and interpersonal skills.
• Ability to prioritize tasks and manage multiple responsibilities.
• Proficiency in EMR systems and strong computer skills.
• Compassionate, patient-centered approach with a focus on dignity and respect.

Compensation & Schedule

• Pay: $16–$18 per hour
• Bonus potential: Up to $5,000 per month
• Schedule: Monday–Friday, 8:30 AM – 5:00 PM (30-minute lunch)
• No weekends required
