About Us:
Hazelwood House is a small non-profit organization dedicated to providing day programming for adults with developmental disabilities. Our mission is to enrich the lives of our participants by removing obstacles, fostering meaningful connections, and creating a safe and supportive environment where all abilities are celebrated.
Position Overview:
We are looking for a dedicated and compassionate Day Program Support Worker to join our team. This position is contracted for 12 - 16 months starting in September 2025 and involves working Tuesday-Friday from 9:00 am to 4:00 pm.
In this role, you will actively support participants, assist with their personal care and support needs, and help with daily operations. We are seeking someone who thrives in a small team environment and enjoys staying active and engaged throughout the day.
Key Responsibilities:
Facilitate Programming: Engage participants in various activities, including socialization, exercises, and group activities, while promoting personal development and enhancing social and daily living skills.
Community Engagement: Support participants during community outings, ensuring they have positive experiences and facilitating their interaction within the community.
Support Needs: Understand and accommodate individual support needs, including conditions like asthma or seizure disorders, and follow specific care protocols as required.
Personal Support: Provide assistance with personal care needs, including bathroom help and dressing.
Emergency Response: Be prepared to respond to emergencies or unexpected situations, following established protocols to ensure participants' safety and well-being.
Meal Assistance: Help with meal preparation, accommodate dietary restrictions, and be aware of allergies.
Proactiveness: Anticipate and address participants’ needs and potential issues before they arise.
Follow Routines: Adhere to established routines to provide consistency and stability for participants.
Record Keeping: Track attendance and manage records using an app, completing documentation to keep records accurate and up-to-date.
Cleaning: Perform routine cleaning and organization to keep the program space safe, tidy, and welcoming.
Expectations:
Punctuality: Arrive on time for all scheduled shifts.
Attire: Wear comfortable, appropriate clothing and the Hazelwood House uniform (t-shirt/sweater).
Professionalism: Treat participants, parents, and the public with respect at all times.
Meetings: Attend all required team meetings.
Fundraisers: Participate in and support fundraising events organized by Hazelwood House.
Notice: Provide advance notice if you are unable to work.
Health & Safety: Follow all health and safety protocols and guidelines.
Qualifications:
Experience: Previous experience working with individuals with developmental disabilities or other vulnerable populations.
Background Check: Clean police check with vulnerable sector check.
Certifications: First Aid and CPR Level C certification.
