Compass Community Health offers primary care health services and health promotion programs to diverse communities and individuals.

We are currently seeking a:

Diabetes and Health Promotion Nurse Educator

Permanent Full Time - 37.5 hours/week

Annual Salary Range: $66,311 - $78,009

What we are looking for:

• Registered Nurse (RN) in good standing with the College of Nurses of Ontario (CNO).
• Certification in Diabetes Education (e.g., Certified Diabetes Educator [CDE]) through a recognized program.
• Knowledge of and sensitivity to the impact of social, economic, environmental, and cultural factors in diabetes management and chronic disease prevention.
• Demonstrated understanding of health promotion, disease prevention, and community development principles.
• Proficiency in group facilitation, client education, and community outreach.

What you can expect to do:

• Provide in-house, virtual, and outreach education and monitoring of diabetes and prediabetes, using Diabetes Canada best practice guidelines as a template.
• Administer vaccinations within scope of practice and improve client access to immunization services through outreach, coordination, and culturally sensitive education to address vaccine hesitancy, misconceptions, and barriers to immunization for priority populations.
• Promote and co-facilitate Chronic Disease Self-Management and prevention-focused groups at the Centre and in the community.
