As part of the Renal Supportive Care Service at SWSLHD, you will have the opportunity to make a meaningful difference in the lives of people living with advanced kidney disease while contributing to service excellence and innovation. In this role, you will:
- Deliver high‑quality, patient‑centred nutrition care to individuals with complex needs, supporting symptom management, quality of life and shared decision‑making.
- Work collaboratively within a multidisciplinary team across the continuum of renal and supportive care.
- Apply advanced clinical dietetic expertise to manage a specialised caseload within renal supportive care.
- Contribute to quality improvement, service development and evaluation to meet evolving patient and service needs.
- Support education, supervision and professional development activities, sharing your expertise with colleagues and learners.
Are You the Right Fit?
You are an experienced and compassionate Dietitian who is motivated to work in a specialist renal supportive care setting, delivering thoughtful, patient‑centred nutrition care to people with advanced kidney disease. In this role, you will:
- Provide specialist nutrition assessment and intervention for patients with advanced chronic kidney disease, supporting patients and families with individualised, goals‑of‑care‑aligned nutrition strategies.
- Work closely with nephrology, nursing and allied health colleagues within a multidisciplinary supportive care model.
- Contribute to service development, quality improvement and evidence‑based practice initiatives within the Renal Supportive Care Service.
- Support education, supervision and capacity building across the renal and dietetics workforce.
- Uphold a strong commitment to safe, high‑quality and compassionate care across the continuum of supportive and renal care.
Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.
- Bachelor in Nutrition and Dietetics or Post graduate qualification giving eligibility for full membership of the Dietitians Association of Australia.
- Demonstrated advanced clinical skills in the assessment and management of patients with complex conditions, including chronic kidney disease and/or those requiring supportive or palliative care, with the ability to deliver evidence-based, patient-centred nutrition interventions aligned to goals of care.
- Proven ability to lead or contribute to quality improvement, service development, and evaluation activities, including the implementation of evidence-based practice and innovations that enhance patient care and service delivery within a specialised clinical area.
- Demonstrated highly developed communication and interpersonal skills, with the ability to effectively engage patients and families in shared decision-making, and collaborate within multidisciplinary teams to deliver coordinated, holistic care in a supportive care model.
- Experience in providing clinical supervision, mentoring, and education to dietitians, students, and/or multidisciplinary staff, and contributing to workforce development and capability building within a specialised service.
