Salary

From $38.00 per hour plus Superannuation and applicable SCHADS Award penalty rates and allowances.

Classification and remuneration will be determined in accordance with the SCHADS Award based on qualifications, experience and the inherent requirements of the role.

EddenRose Community Services is a registered NDIS provider expanding specialist Supported Independent Living (SIL) services across South East Melbourne.

We are building a specialist workforce to support adults with complex physical disabilities, neurological conditions and high support needs who require structured, clinically aligned supports within Supported Independent Living and community settings.

Our model focuses on safety, dignity, independence and meaningful participant outcomes through multidisciplinary collaboration and evidence-based practice.

About the Role

As a Disability Support Worker, you will support adults with complex physical disabilities and neurological conditions within Supported Independent Living (SIL), participants' homes and community settings.

You will work collaboratively with Registered Nurses, allied health professionals, Team Leaders and Support Coordinators to deliver supports in accordance with participant support plans, organisational policies and delegated clinical guidance where appropriate.

Participant support requirements vary depending on individual goals, approved NDIS funding, assessed risks and organisational service capability.

Employees are expected to work within the scope of their role, qualifications, training, assessed competency and delegated authority at all times.

Participants You May Support

Participants may include adults living with:

  • Spinal cord injury
  • Multiple Sclerosis (MS)
  • Motor Neurone Disease (MND)
  • Parkinson's disease
  • Cerebral palsy
  • Acquired Brain Injury (ABI)
  • Stroke-related disability
  • Progressive neurological conditions
  • Complex mobility impairment
  • Participants transitioning from hospital or rehabilitation into community-based supports

Supports are delivered only where they align with participant goals, assessed needs, identified risks and EddenRose's service capability.

Key Responsibilities

Responsibilities may include:

  • Personal care and activities of daily living
  • Safe manual handling and transfers using specialised equipment
  • Hoist transfers
  • Community participation and transport
  • PEG feeding following competency assessment and delegated clinical guidance
  • Dysphagia and mealtime support
  • Urinary catheter support
  • Complex bowel care
  • Pressure care and skin integrity management
  • Medication assistance in accordance with organisational policy and scope of practice
  • Clinical observations and reporting changes in participant condition
  • Accurate progress notes and documentation
  • Incident reporting and risk escalation
  • Promoting participant dignity, choice and independence

Our Service ModelComplex Clinical Supports

Support participants with complex physical disabilities and neurological conditions in accordance with participant support plans, delegated clinical guidance, organisational policies and the NDIS Practice Standards.

Clinical Collaboration

Work collaboratively with Registered Nurses, occupational therapists, physiotherapists, speech pathologists, psychologists, Support Coordinators and other allied health professionals to promote coordinated, safe and consistent outcomes.

Structured Supported Independent Living

Deliver supports within structured Supported Independent Living environments that promote participant safety, independence, dignity and quality of life.

Professional Practice

Maintain accurate documentation, communicate effectively, identify and report incidents or changes in participant condition, maintain professional boundaries and perform duties within the scope of your role, training and assessed competency.

Essential Requirements

Applicants should have:

  • Experience supporting adults with complex physical disabilities, neurological conditions or high support needs.
  • Experience within disability support, Supported Independent Living (SIL), rehabilitation, hospital transition or community care.
  • Current NDIS Worker Screening Check (or willingness to obtain).
  • Current First Aid and CPR certification.
  • Current Australian Driver Licence.
  • Reliable motor vehicle where transport forms part of the role.
  • Completion of the NDIS Worker Orientation Module.
  • Strong written and verbal communication skills.
  • Excellent documentation skills.
  • Current Right to Work in Australia.

Highly Regarded Experience

Experience supporting participants with one or more of the following is highly regarded:

  • PEG feeding
  • Dysphagia and mealtime management
  • Urinary catheter support
  • Complex bowel care
  • Pressure injury prevention
  • Hoist transfers
  • Neurological conditions
  • Spinal cord injury
  • Hospital transition or rehabilitation support
