EMBRACE is a clinical program developed to give Black mothers and Black pregnant people an opportunity to receive integrative/collaborative perinatal care from an intentional angle of racial consciousness.
EMBRACE is focused on closing the gap for Black maternal and Black infant birthing disparities and reducing inequitable care for a birthing population at risk.
The purpose of this Perinatal Patient Navigator position is to provide non-clinical behavioral health support, guidance and assistance to Black adult patients that are pregnant or who have recently given birth as part of a racial-concordant interdisciplinary team utilizing an Integrated or Collaborative Care Model. This position will support patient navigation through complex healthcare environments. Primary goals are to increase patient satisfaction, and to optimize care and outcomes. Acts as a communication liaison to understand the patient's non-clinical individual needs, desires, and concerns. Guides the patient and family to a broad range of services, amenities, and information to promote healing and ensure satisfaction with the patient care experience.
This position seeks candidates from backgrounds that represent the targeted population being served; who have perinatal behavioral health expertise and demonstrated commitment to fostering anti-racist practice, racial justice, and behavioral health equity; who will help create more accessible, equitable, coordinated, culturally responsive and person- centered care. This position will help program leadership accomplish the following goals:
- Implementation of new outreach and engagement practices for pregnant/expecting Black/African American clients including those who are currently underserved by the County medical plan.
- Implementation of culturally adaptive perinatal interventions and practices that increase client satisfaction, efficacy, and retention.
- Implementation of perinatal cultural practices that emphasize elements of the Sankofa framework
The final salary and offer components are subject to additional approvals based on UC policy.
Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $29.99 - $48.23 (Hourly Rate).

Department Description
Required Qualifications
- Bachelor's degree in related area and / or equivalent experience / training
- Minimum 1+ years’ experience and knowledge of working with people during the perinatal phase (pregnancy and postpartum), maternal and infant health/well-being
- Experience with community resource coordination
- Knowledge of mental health, social, and cultural intervention needs of Black/African American communities
- Ability to work independently in a fast-paced, demanding environment with minimal supervision.
- Ability to manage and oversee multiple tasks simultaneously and exert strong time management skills
- Ability to communicate effectively, both orally and in writing
- Proficiency with Windows-based software including Microsoft word, Excel, Outlook. Knowledge of computer systems and software used in functional area
- Strong skills to comprehend and assess patient's grievances to quickly locate appropriate resource for assistance. In-depth knowledge of the organization and how to get issues resolved
- Strong interpersonal and customer service skills. Ability to communicate and resolve issues effectively with a diverse population of patients, staff and physicians
- Excellent analytical and problem-solving skills. Ability to develop solutions and recommend changes and follow through with implementation
Preferred Qualifications
- Minimum 2 years’ experience and knowledge of working with people during the perinatal phase (pregnancy and postpartum), maternal and infant health/well-being
- Community Health Worker (City College of San Francisco Homeless Prenatal Program) certification
- Birth/Labor Doula Verified Training/Certification
- Lactation Consultation Certification
- Ability to work flexible hours, have reliable transportation and the ability to travel throughout the city of San Francisco
- Experience working in hospital/medical setting
- Experience working as a doula or in-home visitor providing social services to a diverse population

- Previous UC experience
- Previous experience with Epic EMR
- Previous experience working with anxiety, depression, trauma, and Serious Mental Illness populations
- Acquiring knowledge of Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare / Medicaid regulations. Knowledge of Medical Terminology. Knowledge of data collection, compilation, and analytical techniques.
