Primrose Hospice is growing—and we’re looking for a relationship-driven Community Relations Hospice Liaison who is passionate about connecting patients and families to compassionate end-of-life care.

What You’ll Do

• Build and grow referral relationships with physicians, hospitals, SNFs, and assisted living communities
• Educate partners on hospice services, eligibility, and philosophy
• Represent Primrose Hospice in the community with professionalism and integrity
• Support census growth through strategic outreach and relationship development
• Coordinate community events, presentations, and networking opportunities
• Track referral activity and collaborate with leadership on growth initiatives

What We’re Looking For

• Experience in healthcare marketing, hospice, home health, or clinical outreach preferred
• Strong relationship-building and communication skills
• Self-starter who thrives in a fast-paced, people-centered environment
• Knowledge of hospice or end-of-life care is a plus
• Valid driver’s license and ability to travel locally
