Job Overview
We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will provide comprehensive care to patients of all ages, focusing on health promotion, disease prevention, and management of acute and chronic illnesses. As an FNP, you will work collaboratively with physicians and other healthcare professionals to deliver high-quality patient-centered care in a supportive environment.
Responsibilities
- Conduct thorough patient assessments, including physical examinations and health histories.
- Develop and implement individualized care plans based on patient needs.
- Provide education to patients and families regarding health management, disease prevention, and wellness.
- Perform diagnostic tests and interpret results to guide treatment decisions.
- Administer medications and treatments as prescribed while ensuring adherence to aseptic techniques.
- Engage in discharge planning for patients transitioning from hospital to home care.
- Collaborate with multidisciplinary teams to coordinate patient care and ensure continuity of services.
- Maintain accurate patient records in compliance with HIPAA regulations.
- Participate in utilization review processes to enhance the quality of care delivered.
- Stay current with medical advancements and best practices in family medicine.
Qualifications
- Master’s degree in Nursing with a focus on Family Nurse Practitioner studies.
- Current certification as a Family Nurse Practitioner from an accredited organization.
- Valid state nursing license with prescriptive authority.
- Strong knowledge of anatomy, medical-surgical nursing, and pediatric care including infant and toddler care experience.
- Familiarity with discharge planning processes and utilization review methodologies is preferred.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
- Ability to work in a teaching hospital environment is a plus.
