Supervises: No supervisory responsibility

Job Summary:

As a member of the inter-disciplinary team and under the supervision of the Resident & Community Services, the FSS Coordinator provides services designed to assist those individual residing in Public Housing and Section 8 with the coordination of a wide variety of social services, consistent with the requirements and standards of the HUD, IHCDA, and its contract with the Housing Authority of the City of Terre Haute.

This is a grant funded, full time professional position in the Resident and Community Services Department of the Housing Authority if the City of Terre Haute. The Resident and Community Services Department is committed to the respectful provision of services and education that offer opportunities for participants of our subsidized housing programs to increase their housing and financial stability, promote completion of self-established goals, and achieve the future they have planned for their family.

The FSS Coordinator provides classroom and individualized training in the areas of employment readiness, financial literacy, self-empowerment, goal setting, and preparation for homeownership. The FSS Coordinator also provides individualized case management to FSS participants; including but not limited to: performing needs assessments, development of service plans, linking to appropriate services, identifying goals, and ongoing monitoring of participant progress. The FSS Coordinator is also responsible for maintaining FSS client files, completing reports as required, and remaining in compliance with the FSS grant, HUD regulations and THHA policies and procedures.

This position works with clients, community social service providers, employment service providers, and inter-disciplinary team within THHA. Additional duties may be assigned by the Director of Housing.

Essential Functions May Include but Not Be Limited To:

1. Maintain knowledge of the FSS Program and its requirements and standards.

2.Ensure that the rights of persons served are consistently maintained.

3. Conduct client identification and recruitment activities.

4. Determine and document client eligibility, consistent with program regulations.

5. Conduct client assessment, intake, and orientation activities.

a) Complete Initial Intake (assessment)

b) Develop training classes for participants

6. Develop and implement the Individual Training Services Plan (ITSP) consistent with program standards.

7. Coordinate the ISTP with referral sources, community agencies, and family members/significant others, as appropriate.

a) Provide linkage, coordination and facilitate introductions with the client to Community Resources.

8. Monitor and evaluate the individualized ITSP in the time frames required by internal and external sources.

a) Complete Monthly Report every 30 days

b) Complete annual reports

9. Monitor and evaluate service utilization, documenting the status of each person served.

10. Conduct crisis intervention services, as necessary.

11. Conduct discharge planning and case closure activities in accordance with program requirements.

12. Ensure that all funding source reporting, documentation, and compliance activities are conducted in a timely manner.

13. Ensure timely and cooperative responses to all local agencies and HUD requests, communications, and referrals.

14. Marketing and presentation of the programs to meet target goals of enrollment.

15. Attend local meetings that would be required or could benefit the program.

16. Maintain communication directly with Program Administrator to assist in gathering needed information for HUD/IHCDA reports.

17. Coordination of services for other Housing Opportunities.

18. Develop and provide monthly newsletter to participants.

19. Attend all internal and external training/professional development sessions and administrative meetings.

20. Conduct advocacy services for persons served.

21. Adhere to and promote the Corporate Compliance Program of the Housing Authority of the City of Terre Haute.

Additional Responsibilities:

1.Available to work some evening and weekend hours to assure that participant needs are met.

2. May require use of personal vehicle.

3. All other duties as assigned.

Specifically, provide information and referral resources in the following areas and others as pertinent to the resident population and resident need:

  • THHA programs
  • Parenting, child care programs, after school programs; family and youth counseling
  • Mental health
  • Job training/Employment Readiness/Education
  • Legal aid
  • Drug and alcohol abuse
  • Elder-care services
  • Government entitlement programs

Training and Development:

Participates in continuing education programs and in-service training on request or in conjunction with developmental and training goals or as deemed appropriateto increase personal and professional effectiveness and improve the service quality provided our customers.

Policies and Procedures:

Will support and demonstrate through his/her actions the philosophy of the Housing Authority of the City of Terre Haute as well as its corporate mission, vision, and values. Individual will meet all documentation requirements of the organization and of its funding sources.

Education and/or Experience:

BS, BSW, or an MS degree in sociology, psychology, counseling, pastoral counseling, family and child development; or a bachelor’s or master’s degree in another field which includes a minimum of twelve hours of course work in human behavior, social policy, social welfare, or counseling; and one year of experience (full-time) in case management, including volunteer and intern positions.

If this experience is lacking, the individual must be supervised by an experienced care coordinator for a period of one year. This supervisor is required to review and initial the individual’s work during this period.

Knowledge, Skills & Abilities:
Must demonstrate ability to teach and model daily living skills and vocational skills;

Must demonstrate knowledge of community resources and interagency linkages;

Must demonstrate possession of refined assessment, verbal and written communication, and interpersonal skills;

Must demonstrate an ability to learn and implement the principles and concepts of psychosocial rehabilitation;

Ability to demonstrate good judgment and decision-making independent of the need for direct supervision (although subject to review) and written protocols;

Ability to provide courteous customer service to consumers and other staff members;

Must demonstrate knowledge and interest of working with participants with persistent mental illness, addictions, and/or disabilities;

Must demonstrate a willingness to work in a performance improvement environment as a member of a high performance work team; and ability to follow corporate guidelines related to personnel policies and procedures.

While performing the duties of this job, the employee is regularly required to sit and talk or hear. The employee frequently is required to use hands to finger, handle, or feel objects, tools, or controls and reach with hands and arms. The employee is occasionally required to stand; walk; and stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, depth perception, and the ability to adjust focus.

The noise level in the work environment is moderate.
