Social Worker – Grade 2
Community Therapy Service – Pain Clinic and Amputees

- Join our collaborative and experienced social work team in Community Therapy Services (CTS).
- Ongoing, full time (with ADOs) based at the Royal Melbourne Hospital Royal Park Campus.

Your New Role:
- Provide exemplary social work service across but not limited to the Community Therapy Service & Pain Clinic, including psychosocial assessment, emotional support and counselling, and discharge planning.
- Demonstrate excellent communication strategies with patients, relatives, staff, and relevant services in order to provide appropriate services and information.
- Liaise with service providers and refer to relevant community services, Aged Care Assessment Service and Transition Care Program.
- Enjoy the leadership and support of senior clinicians (grade 3) and clinical leads (grade 4) as well as the CTS multi-disciplinary team.
About You:
- You have demonstrated health & social work experience with a collaborative working style.
- You have excellent communication skills and a passion for continued professional development for yourself and others.
- You are a qualified Social Worker, eligible for membership with the Australian Association of Social Workers.
- Grade 2 applicants should have at least 3 years’ experience.
- You value working with supportive and dedicated health professionals and multidisciplinary teams.
