Duties
- Provide clinical leadership and supervision to the nursing and personal care teams, ensuring the delivery of safe, high-quality, resident-centered care.
- Oversee resident assessments, individualized care plans, medication management, and ongoing clinical monitoring to meet each resident's changing needs.
- Ensure compliance with the Retirement Homes Act (RHA), RHRA standards, infection prevention and control (IPAC) requirements, and all applicable provincial regulations.
- Collaborate with physicians, nurse practitioners, pharmacists, hospitals, Ontario Health atHome, and community partners to coordinate resident care and support seamless transitions.
- Lead the recruitment, orientation, scheduling, coaching, and performance management of clinical staff while fostering a positive and accountable workplace culture.
- Monitor quality indicators, conduct clinical audits, investigate incidents, and implement continuous quality improvement initiatives to enhance resident outcomes and regulatory compliance.
- Support occupancy growth by participating in resident assessments, admissions, family meetings, and interdisciplinary care planning while promoting an exceptional resident and family experience.
Requirements
- Current registration and in good standing with the College of Nurses of Ontario (CNO).
- Minimum 3–5 years of progressive clinical leadership experience in a retirement home, long-term care, or seniors' healthcare setting.
- Demonstrated experience leading memory care and dementia care programs, with a strong understanding of person-centred care, responsive behaviours, and cognitive support strategies.
- Thorough knowledge of the Retirement Homes Act (RHA), RHRA regulations, infection prevention and control (IPAC), medication management, and quality assurance standards.
- Proven ability to recruit, mentor, coach, and lead multidisciplinary healthcare teams while fostering a positive, accountable workplace culture.
- Experience coordinating care with physicians, nurse practitioners, hospitals, Ontario Health atHome, Behavioural Supports Ontario (BSO), Alzheimer's Society, and other community healthcare partners.
- Strong clinical knowledge in resident assessments, care planning, medication management, chronic disease management, palliative care, and end-of-life care.
- Experience with electronic health records and clinical documentation systems.
- GPA (Gentle Persuasive Approaches) certification or coach designation.
- PIECES™ training and experience implementing dementia care best practices.
- Experience developing and leading specialized memory care programs.

Pay: $70,000.00-$80,000.00 per year
