Health Coach – Orthopedic & Geriatric Focus

Position Summary

The Health Coach will play a pivotal role in supporting patients with musculoskeletal (MSK) and orthopedic concerns, particularly within the geriatric population. This individual will leverage their clinical knowledge to guide patients through conservative care options, foster strong relationships with both patients and clinical teams, and coordinate care to optimize health outcomes before and after surgery. The ideal candidate is a self-starter, highly relationship-driven, and motivated by data and goal achievement.

Key Responsibilities

  • Build and maintain strong, trusting relationships with patients, surgeons, and clinical staff to ensure seamless care coordination and high engagement throughout the MSK journey.
  • Conduct initial and ongoing assessments of patients’ health status, functional goals, and readiness for conservative management.
  • Educate patients on the importance of conservative care options (physical therapy, exercise, nutrition, injections) and support their adherence to these interventions prior to considering surgery.
  • Engage patients in tailored physical activity programs and lifestyle modifications to improve musculoskeletal health and overall well-being.
  • Make regular follow-up calls to coach patients, monitor progress, address barriers, and provide motivation and accountability.
  • Collaborate closely with surgeons and clinical staff to ensure coordinated, patient-centered care and to communicate patient progress and concerns.
  • Document all patient encounters, track outcomes, and use data to drive continuous improvement in patient engagement and health results.
  • Demonstrate a high sense of ownership and initiative in managing a patient panel, adapting strategies as needed to achieve optimal outcomes.

Required Qualifications

  • Clinical background or strong knowledge in musculoskeletal and geriatric health (e.g., physical therapy, nursing, exercise physiology, or related field).
  • Experience working with orthopedic or geriatric populations, preferably in a coaching, care coordination, or patient education role.
  • Excellent interpersonal, communication, and motivational skills; able to build rapport and trust with diverse populations.
  • Goal-oriented, data-driven, and comfortable working independently to achieve measurable patient outcomes.
  • Strong organizational skills and ability to manage multiple patient relationships and tasks simultaneously.

Preferred Qualifications

  • Certification in health coaching or related field.
  • Experience with digital health tools and remote patient engagement
  • Familiarity with conservative orthopedic interventions and pre/post-surgical care pathways.

Pay: From $50,000.00 per year
