Health Educator/Promoter

Full-Time | Maternity Leave Coverage

The Athens District Family Health Team, located in beautiful Eastern Ontario between the historic Rideau Waterway and the world‑famous Thousand Islands, is seeking a full-time Health Educator/Promoter to join our team in a maternity leave coverage role.

We are more than just a clinic — we are a small, supportive, and genuinely close‑knit team that values collaboration, respect, and caring for one another. You will work alongside physicians, nurse practitioners, nurses, allied health professionals, and dedicated administrative staff who believe that a positive workplace culture is just as important as delivering excellent patient care.

Reporting within our multidisciplinary team, the Health Educator/Promoter plays a key role in supporting prevention, early intervention, and chronic disease management for our patients. This is a unique and flexible position, with responsibilities that can be adapted to reflect the successful candidate’s skills, experience, and professional interests.

In this role, you will develop, coordinate, and deliver health promotion and education programs, and provide disease management education and health teaching to individuals and groups served by the Athens District Family Health Team. You will collaborate closely with clinical colleagues to support patient‑centred care that promotes long‑term health and well‑being.

The ideal candidate has strong computer skills, including proficiency with Excel, Word, and PowerPoint, and is comfortable compiling statistics and analyzing outcome data from health programs. Previous experience using an electronic medical record (EMR) system is considered an asset.

Applicants with a clinical or educational background in one or more of the following areas are encouraged to apply:

  • Primary or community health nursing
  • Diabetes education
  • Degree in Health Promotion
