This position is a direct care position responsible for the scheduling, safe transportation and accompaniment related to medical appointments for as determined necessary, and health-related duties for Caroline Center, Inc. residents. The HSL position includes clerical supports for assigned Program Participants as related to health services and Person-Centered-Plans (PCPs).

Contributes to effective and efficient communication within HCPs and CCI staff Schedule, transport and accompany to and from medical appointments that are classified as emergency and/or non-routine, (ex. initial consults and new referrals) for residential participants

Participate in residential team meetings

Report observations and services delivered according to agency procedures

Record service delivered in CCI approved formats and platforms

Utilizes existing resources and reports to promote compliance and efficient operations

Participates in On-call rotation as assigned, and provides direct supports as needed

Accurately completes incident reports per agency procedures

Accurately conveys information between nursing staff, treating HCPs and CCI program staff

Communicates health care information effectively, efficiently and professionally between CCI employees and participant’s treating HCPs

Advocates for Participants to receive health care services comparable to those provided to people without disabilities

Maintains trackers developed to ensure timely completion of various nursing duties

Participates in the process of accepting and reviewing PCPs in LTSS

Prepares paperwork in advance of healthcare appointments

Processes paperwork following healthcare appointments in accordance with PnP

Prepares PCP packets in advance of annual meetings to include all CCI required forms

Up-dates IDFs in Therap as indicated

Maintains paper records for Participants in accordance with PnP

Purges paper records in accordance with PnP

Completes timely and appropriate documentation regarding medical appointments

Maintains accurate records in Medication Administration Records; completes entries and discontinuations upon receipt of orders

Provides direct care to participants, as needed, in accordance with the PCP

Participates in the functions of agency committees and special projects as assigned

Hours: Mon 8:30a-4p, Tues 8a-3:30p, Wed. & Thur 8a-4p Fri. 3p- 12midnight; on-call rotation includes weekends

Job Type: Full-time

Pay: $18.75 - $23.00 per hour
