We are seeking a compassionate and dedicated Home Care Nurse to provide leadership to a team of HCA's. High-quality patient care in the comfort of our clients' homes. The ideal candidate will have a strong background in nursing, scheduling, Client assessment, problem-solving, and intake assessment. This role requires excellent communication skills, problem-solving, and a commitment to enhancing the quality of life for patients.
Responsibilities
- Conduct comprehensive assessments of patients' health status and develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' vital signs and overall health, reporting any changes to the healthcare team.
- Adjusting and planning schedules as needed
- Provide education and support to patients and their families regarding health management and wellness.
- Assist with and oversee the delivery of daily living activities, including personal hygiene, mobility, and nutrition.
- Maintain accurate medical records and documentation in compliance with healthcare regulations.
- Collaborate with other healthcare professionals to ensure coordinated care for patients.
Skills
- Strong knowledge of patient care practices and medical terminology.
- Excellent interpersonal skills with the ability to build rapport with patients and families.
- Proficient in nursing procedures and techniques, demonstrating a high level of clinical competency.
- Ability to work independently while managing time effectively to meet patient needs.
- Compassionate attitude towards patient care, ensuring dignity and respect at all times.
- Strong problem-solving skills, with the ability to adapt care plans as necessary based on patient conditions.
Job Types: Part-time, Permanent
Pay: $26.00-$37.00 per hour
