Job Summary
Join our dedicated team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their own homes. This position offers an exciting opportunity to make a meaningful difference in the lives of patients by assisting with daily activities, monitoring health status, and supporting overall well-being. As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver high-quality care that promotes independence and comfort. This role is ideal for caring, energetic individuals eager to bring warmth and professionalism into home health settings.
Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patient health status, including vital signs and overall condition
- Support medication administration following proper protocols and medical documentation standards
- Maintain accurate records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document care activities and patient progress
- Provide specialized care such as Alzheimer's, dementia, memory care, hospice support, catheter care, and working with individuals with developmental disabilities or disabilities
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment
- Utilize equipment like Hoyer lifts safely and effectively to assist with patient transfers while adhering to HIPAA regulations and safety standards
Skills
- Strong patient observation skills to detect changes in health or behavior promptly
- Experience in assisted living facilities, nursing homes, or home health environments
- Knowledge of medical terminology and medical documentation practices
- Ability to work with EMR/EHR systems efficiently for recordkeeping
- Proficiency in assisting with ADLs and providing compassionate senior care or home & community care
- Familiarity with hospice care protocols, dementia care strategies, and memory care techniques
- Certification in CPR and training in catheter care or other specialized medical procedures preferred
- Excellent communication skills for working effectively with patients, families, and healthcare teams
- Ability to handle sensitive situations with empathy while maintaining confidentiality under HIPAA guidelines
