Job Summary
Join our dedicated team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their own homes. As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver high-quality care that promotes independence and comfort. This role is ideal for caring, energetic individuals eager to bring warmth and professionalism into home health settings.

Responsibilities

  • Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Monitor and observe patient health status, including vital signs and overall condition
  • Support medication administration following proper protocols and medical documentation standards
  • Maintain accurate records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document care activities and patient progress
  • Provide specialized care such as Alzheimer's, dementia, memory care, hospice support, catheter care, and working with individuals with developmental disabilities or disabilities
  • Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment
  • Utilize equipment like Hoyer lifts safely and effectively to assist with patient transfers while adhering to HIPAA regulations and safety standards

Skills

  • Strong patient observation skills to detect changes in health or behavior promptly
  • Experience in assisted living facilities, nursing homes, or home health environments
  • Knowledge of medical terminology and medical documentation practices
  • Ability to work with EMR/EHR systems efficiently for recordkeeping
  • Proficiency in assisting with ADLs and providing compassionate senior care or home & community care
  • Familiarity with hospice care protocols, dementia care strategies, and memory care techniques
  • Certification in CPR and training in catheter care or other specialized medical procedures preferred
  • Excellent communication skills for working effectively with patients, families, and healthcare teams
  • Ability to handle sensitive situations with empathy while maintaining confidentiality under HIPAA guidelines
