ABOUT THE ROLE
We're looking for a skilled and compassionate Registered Nurse to join our home health team on a per diem basis. In this role, you'll provide direct skilled nursing visits — including Start of Care (SOC) admissions, routine skilled visits, and recertifications — for patients throughout Southern Palm Beach County. WHAT YOU'LL DO

• Conduct Start of Care (SOC) and admission visits, including comprehensive OASIS assessments
• Perform ongoing skilled nursing visits per physician-ordered plan of care
• Assess patient condition, symptoms, and progress at each visit
• Administer treatments and skilled nursing interventions per orders (wound care, injections, catheter care, etc.)
• Educate patients and caregivers on diagnoses, medications, and home care techniques
• Document all visits accurately and timely in the agency EMR (WellSky Kinnser) in accordance with Medicare/Medicaid and agency standards
• Communicate patient status changes promptly to the case manager, DON, and physician
• Follow all Florida state home health regulations and agency policies

WHAT WE'RE LOOKING FOR

• Active Registered Nurse (RN) license in good standing with the Florida Board of Nursing
• OASIS/SOC documentation experience required
• Experience with WellSky Kinnser EMR preferred
• Current CPR/BLS certification
• Valid Florida driver's license, reliable transportation, and current auto insurance
• Strong clinical documentation and communication skills
• Ability to pass a Level 2 background screening and provide current health screening (TB test/immunizations)
• Comfortable working independently and managing your own visit schedule

SCHEDULE & PAY

• Per diem — pick up visits based on your availability
• Paid per visit
• Service area: Southern Palm Beach County

WORK ENVIRONMENT
This position involves travel to patient homes throughout the service area. The role requires the ability to lift and maneuver as needed, communicate clearly, and operate standard clinical equipment. Reasonable accommodations are available.

Work Location: On the road
