Job Overview
We are seeking a dedicated and compassionate rural homebound client coordinator to join our team. In this role, you will be responsible for managing client relationships, adhering to program requirements, and ensuring that nutrition and other services are delivered effectively to homebound older adults. The ideal candidate will possess strong administrative, time management, and telephone skills, as well as a passion for supporting vulnerable older adults in the agency's planning and service area.

Responsibilities

  • Develop and maintain positive relationships with clients and their families to ensure a high level of satisfaction and engagement.
  • Coordinate activities to ensure the effective delivery of available services.
  • Maintain all client records to be in compliance with program requirements.
  • Provide administrative support by maintaining accurate records, documentation, and reports related to client progress.
  • Collaborate with interdisciplinary teams to assess client needs and develop personalized service plans.
  • Monitor program effectiveness and implement improvements as necessary.
  • Advocate for clients’ rights and assist them in accessing community resources.
  • Conduct regular evaluations of client programs to ensure compliance with organizational standards.

Requirements

  • Experience with social work or community health care work, or a related field preferred but not necessary. Minimum of 2 years office and computer experience with a high school diploma
  • Experience working with vulnerable populations is highly desirable.
  • Strong skills in relationship management and program management.
  • Proficiency in administrative tasks.
  • Excellent communication skills, both verbal and written.
  • Ability to work in a collaborative team environment.
  • Strong organizational skills with the ability to manage multiple priorities simultaneously.

Job Type: Full-time

Pay: $17.50 per hour

Expected hours: 40 per week
