About the role
The Case Management Worker position within the Homelessness team works collaboratively with the Supported Referral Program as part of the Northern Metropolitan Homelessness Network. The role applies case management practices in line with program guidelines and practice standard and follows continuous quality improvement processes for people experiencing homelessness or at risk of homelessness.
As a Case Manager, you will support clients by:
Delivering case management that aligns with organisational and state practice standards;
Providing timely and appropriate assessment, consumer engagement, outreach, intervention, care coordination, needs identification, resource assessment, supported referral, advocacy, information provision, and planned disengagement/ discharge;
Developing client-centred care plans with consumers and relevant stakeholders, identifying goals and supporting self-management.

About you
To be successful, you will bring demonstrated case management experience in the homelessness sector or other relevant community settings, along with:
A relevant Australian tertiary qualification in social work, community services, or a related discipline;
Knowledge of the Specialist Homelessness Services (SHS), Supported Accommodation Assistance Program, homelessness service system, and current developments, including DHHS Standards, continuous quality improvement, and the Opening Doors Framework.
