The main purpose of this position
As the House Support Coordinator, you create and maintain a safe, clean and welcoming environment for the people that we provide support to within our residences. You work onsite within our residences and monitor health and safety; provide supportive counselling; implement house rules, policies and procedures; and respond to crisis situations. You provide a supportive, safe, and welcoming environment for those within the residence.
The House Support Coordinator will support mental health and substance use needs with a special focus on supporting residents experiencing social and physical harms associated with chronic substance use through providing support and resources for harm reduction and prevention.
Our work is to maintain housing stability and tenancy with harm reduction approaches in order to improve people’s wellness.
Responsibilities
Care Focused Work
· Facilitate crisis intervention and de-escalation and respond quickly to support needs.
· Participate in resident care planning through intake assessments, team meetings, and ongoing documentation of client progress.
· Support residents with health and wellness management by reinforcing medication adherence, managing minor health issues, and providing health education.
· Facilitate referrals to health and addiction services and serve as a link to additional health supports as residents’ needs evolve.
· Oversee the operations of the residences while on shift.
· Support residents with mental health concerns or crises utilizing supportive counselling, strength-based approach, crisis intervention skills and engaging appropriate resources as needed.
· Engage residents who use drugs into respectful conversations about safer drug use, safer sex, overdose prevention and reduction of other harms associated with drug use.
· Provide harm reduction and overdose prevention resources
· Monitor drug use (including injections, and inhalation) and respond to overdose with injection naloxone and engagement of emergency and/or crisis services when required.
· Perform resident bed checks.
· Interact with residents consistent with individual needs and plans.
· Provide assistance with congregate living routines, including socialization and activities of daily living (I.e., cleaning, cooking, grocery shopping).
· Will assist residents with household and individual room upkeep during shift.
· Ensure health and safety of home, including ensuring maintenance and cleanliness of shared spaces.
· Provide symptom management and monitoring as related to mental and physical health status and engage additional resources (including clinical supports) as required.
Provide support associated with medications and treatment plans.
· Maintain accurate case record logs and reports
· Additional documentation can include Incident Reports and Health and Safety reporting.
· Regularly consult with the Supervisor, Grace House and Housing Supports to provide feedback and housing updates
Organization Focused Work

· Build partnerships and maintain knowledge of and positive relationships with community partners through relevant external committees and outreach, while increasing awareness of our services with other organizations
· Attend team and organization meetings.
· Support program planning, evaluation, and reporting as necessary.
· Complete accurate and timely workload measuring logs.
· Support the knowledge of other staff and organizations regarding your role/profession by sharing relevant benefits and evidence, guiding standards as necessary etc.
· Timely completion of timesheets (before the 1st and the 15th of the month), mileage (by the 30/31st of the month) and other administrative tasks
· Travelling to community spaces, residences, and workplace meetings as necessary
· Respond to phone and email-based communications and correspondence, internally and externally, in a timely manner.
Professional Development Work
· Aligning our practice with Ontario College of Social Workers and Social Service Workers professional discipline code of conduct and ethics
· Remain current with training and learning opportunities.
· Participate in regular supervision, annual reviews and debriefs with supervisor and team.
· Participate in relevant Communities of Practice
Personal Development Work
· Regularly reflecting upon relevant personal wellness strategies
· Be open and share any work-related needs to the appropriate staff, for the purpose of accommodation, support, and clarity.
· Participate in regularly scheduled breaks and other wellness activities to maintain your work and practice.
· Maintaining your personal wellness - having our own supports and maintaining our wellness, EAP
· Disclose and navigate dual relationships within the organization and with PES, as needed.
Knowledge and skills necessary to be successful in this role
· Post-secondary education in the social services field coupled with relevant experience in community-based mental health services.
· Registered with the Ontario College of Social Workers and Social Service Workers is an asset.
· Proven ability to connect, empathize and work alongside people experiencing Mental Health and substance use concerns
· Experience in harm reduction best practices and overdose prevention
· Proven ability to support someone where they are at
· Crisis intervention and de-escalation skills and relevant trainings
· Exceptional interpersonal skills that promote strong relationships, positive team building, conflict resolution and solution focused problem solving
· Knowledge of recovery-based program philosophy. Proven ability to work from recovery-based and holistic wellness philosophy
· Strong working knowledge of relevant community resources
· Experience using the following tools is considered an asset: OCAN, LOCUS, GAINS
· Excellent written and documentation skills, with the ability to time manage a mix of administration and direct service
· Demonstrated sound judgment, integrity, patience
· ASIST Trainings, NVCI Trainings, etc.
· Proven ability to take initiative and work independently, as well as in a team based environment
· Proven technology proficiency, including expertise with Microsoft Products and ability to work on Electronic Healthcare Record
· Self-awareness, self-reflection, and commitment to ongoing learning and personal development
· A valid driver's license and access to a vehicle
· Current First Aid & CPR certification and Naloxone Training
Working conditions
· Works 40 hours per week with a mix of weekdays, evenings and weekend shifts.
· The position is based out of Support House’s 24/7 congregate living location based in Oakville. Staff will be required to work within all of our housing programs within Halton as required.
Job Type: Full-time
Pay: $57,333.00 per year
