Schedule: Monday, Wednesday, Friday 8:30AM-1:30PM

What You'll Do:

- Assist with personal care, like bathing, toileting, and dressing

- Help with transfers using a stand/pivot method

- Support with meal prep as needed

- Assist with repositioning

- Take care of light housekeeping tasks

- Lead range of motion exercises

What We’re Looking For:

- At least 3 years of caregiving experience

- A reliable attitude and a strong work ethic

- Ability to follow directions easily

- A valid driver's license

Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.

Self-direction allows participants to make their own choices—selecting who provides their care and how those services are delivered.

While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.

Our mission is to help individuals of all ages navigate self-directed care with connection, coordination, and compassion.

Pay: From $16.32 per hour

Expected hours: 18 per week
