Job Overview
We are seeking a dedicated and energetic Information/Referral & Assistance Specialist to join our dynamic team. In this vital role, you will serve as a frontline resource, providing accurate information, compassionate referrals, and comprehensive assistance to individuals seeking support within diverse communities. Your proactive approach will help connect clients with essential community support services, behavioral health resources, and assist with securing benefits through financial support programs. This position offers an exciting opportunity to make a meaningful difference by supporting high-risk populations, including underserved communities, older adults, and individuals with disabilities. Your expertise in client communication, crisis management, and motivational interviewing will drive positive outcomes and foster trust among those you serve.

Responsibilities

  • Provide clear, empathetic information and referrals to clients seeking social services, behavioral health support, and securing benefits through programs including: Medicare, Medicaid, SSI, Food Stamps etc.
  • Answer general questions related to the Case Management Program, Elder Abuse Prevention and Advocacy Program, and the Family Caregiver Program while facilitating referrals for these programs.
  • Assist clients in navigating complex systems such as healthcare, housing, employment, and disability services.
  • Maintain accurate data entry and documentation within client management systems while ensuring confidentiality and compliance.
  • Employ crisis management skills to de-escalate situations and connect clients with immediate support when necessary through administering benefits through material aid funds.
  • Utilize motivational interviewing techniques to encourage client engagement and promote positive behavioral change.
  • Collaborate with community partners and culturally diverse organizations to expand service access and improve outreach efforts.

Experience

  • Proven experience working with individuals from underserved communities or high-risk populations
  • Bachelor's degree in social work, social services, or related fields.
  • Demonstrated ability to work effectively with older individuals or with individuals with disabilities .
  • Excellent written and verbal communication skills for client interaction and report writing.
  • Proficiency in Microsoft Office applications for documentation, data entry, and reporting purposes.
  • Experience conducting client assessments, case management, and providing client-centered support strategies.
  • Knowledge of community resources.
  • Ability to work independently and multitask while working within a team environment.

Pay: $47,000.00 - $52,000.00 per year
