Lcsw - Outpatient

Overview: Responsibilities:

• Participation in departmental rounds or team meetings as required
• Comprehensive bio-psychosocial assessment of patient
• Treatment planning to address issues identified in comprehensive assessment
• Resource linkage and advocacy with appropriately identified resources
• Participation in departmental objectives and assignments
• Outpatient Psychiatry Specific Duties
• Provide psychotherapy on an outpatient basis to individuals, couples, families and/or groups in accordance with NYS Office of Mental Health guidelines and standards of care.
• Complete comprehensive assessments/diagnostic formulations and preliminary treatment plan goals, objectives and methods, within the time frames outlined by regulations (OMH/CMS)
• Provides evidence based therapeutic interventions
• Documents all patient contacts according to NYS Office of Mental Health requirements
• Performs acute/crisis evaluations and coordinates psychiatric hospitalization as needed
• Provides psychoeducation to patients and families
• Functions as a member of the outpatient clinical Treatment Team
• Facilitates referrals for medication evaluations, inpatient psychiatric hospitalizations, and community support services
• Communicates with patient and other treating providers to coordinate treatment plan goals and objectives
• Participates in case conference treatment team meetings
• Treats patients, colleagues, and other staff with cooperation, compassion and respect
• Conducts self in a manner consistent with ethical standards of practice in the field
• Participates in the Utilization Review Process
• Collaborates with outside agencies, such as CPS/DSS
• Performs other duties as requested and observed by supervisor or manager
• Partial remote work is available dependent upon department needs and employee capabilities
  

Qualifications:

• Master's Degree in Social Work, required

• Minimum of 2 years of post Master's Degree experience, preferred Preference for prior related experience

• Current NYS Licensure as an LCSW within 3 months of hire, required

• Sedentary Work - Exerting up to 10 lbs of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Sedentary work involves sitting most of the time. Jobs are sedentary if walking or standing are required only occasionally, and all other sedentary criteria are met

• Occasional bodily fluids/ bloodborne exposure
