General Position Summary:

The Lead Community Health Worker (CHW) serves as a vital link between Johnson Health Center (JHC) and the local community. The individual works with the Director of Population Health to seek opportunities for relationship development with community organizations and potential new patients through outreach activities. The position also provides leadership and direction to Community Health Workers. This team is an integral part of JHC’s patient-centered medical home. The CHW is responsible for providing education to patients, families, and the community, assisting with enrollment, and helping to build and retain JHC patient capacity. CHWs are responsible for collecting information on social determinants of health, assisting with assessment of the data, and helping to address the identified gaps utilizing community partnerships, patient self-management resources, and other tools to serve the ongoing needs of the health center patient population.

Essential Duties and Responsibilities:

1. Provides leadership and direction to Community Health Workers and Spanish Language Interpreter staff.

2. Creates opportunities and collaborates with community partners (including schools, health departments, social services, Head Start, churches and other faith-based organizations, industries, and other community-based organizations) for events and outreach projects to educate the community on JHC’s health care programs, in order to create awareness of the services offered.

3. Attends community functions as a representative of JHC.

4. Maintains ongoing community relationships to identify areas of need for the health center and patients.

5. Monitors patient feedback on the social determinants of health assessment and creates strategies to help alleviate social determinants of health.

6. Maintains certification as a Certified Application Counselor (CAC) and ensures all CHWs are compliant with obtaining certification.

7. Maintains Community Health Worker Certification and ensures all CHWs are compliant with obtaining certification.

8. Develops and maintains a community resource list of available social service agencies and other assistance providers in community for patient referral. Utilize online referral software when necessary.

9. Maintain log of individuals and organizations served and outreach activities conducted.

10. Review patient satisfaction surveys in order to identify areas of improvement and utilize data to implement solutions.

11. Work with the Associate Director of Risk Management (ADRMS) to identify patient complaint trends and identify possible solutions.

12. Review policies to ensure they are culturally appropriate, align with Bridges out of Poverty concepts, and prioritize patient satisfaction.

13. Serves in the CHW role for a designated JHC location.

14. Physical attendance is an essential element of the job and necessary to perform the essential functions of the job.

15. Perform other duties as assigned. This includes and is not limited to JHC hosted events, fundraisers, and specific patient demographic outreach.

Other Functions:

1. Ensure all community outreach activities are conducted in a consistent and professional manner.

2. Must be able to work effectively with all levels of individuals within the community, administrative and professional personnel, and to collaborate with a team to creatively anticipate and solve challenges.

3. Staff will abide by the Code of Conduct as documented in the Corporate Compliance Manual.

4. Must demonstrate a personal and professional commitment to Johnson Health Center and its mission.

5. Must treat all patients and staff with dignity and respect, being mindful of the cultural differences of the diverse population we serve.

6. Management may modify, add or remove any job functions as necessary or as changing organizational needs require.

7. Must be able to successfully complete all required training to become a Certified CHW within one year of employment.

8. Must be able to successfully complete all required training and become a CAC for the health insurance marketplace and Medicaid.

Qualifications:

1. Bachelor’s Degree in social work or health promotion preferred or High School Degree or GED with 4 years’ experience in the field

2. Community Health Worker Certified preferred

3. Successfully complete state-specific as well as federal training to learn how to navigate the health insurance marketplace/Medicaid and become a Certified Application Counselor

4. Demonstrated knowledge of outreach methods, interviewing, and assessment techniques

5. Must possess knowledge of a broad range of social service and assistance providers in the community

6. Demonstrated ability to work as a member of a team and work effectively with community agencies

7. Strong interpersonal communication and relationship-building skills

8. Cultural competency to communicate appropriately with a variety of populations

9. Comfortable with public speaking and leading group discussions and workshops

10. Ability to use computer systems and the Internet

11. Must have a valid driver’s license and reliable insured transportation required

12. Must have an acceptable driving record as demonstrated by a recent printout by the Department of Motor Vehicles.

Physical Demand and Working Environment:

Fast paced office setting with travel to other offices often. Lifting and/or exerting force up to 25 pounds occasionally, with frequently moving of objects. Work requires speaking, sitting, bending, walking, standing, hearing, stooping, kneeling, and repetitive motion with certain activities. Extended hours of constant computer usage. OSHA low risk position.
