  • Basic Nursing Care: Deliver basic nursing and restorative care under the supervision of LPNs and RNs.
  • Communication: Attend report meetings and keep licensed personnel informed of significant observations in resident care and environmental conditions.
  • Personal Hygiene: Assist residents with all aspects of personal hygiene, including bathing, grooming, and dressing.
  • Mobility and Nutrition: Aid residents with ambulation, movement, and nutrition, including feeding, hydration, and toileting.
  • Safety: Ensure a safe environment for residents and staff, responding to emergencies as per facility policy.
  • Care Plans: Understand and follow Resident Care Plans, maintaining confidentiality and privacy.
  • Infection Control: Utilize infection control techniques at all times and respect residents' spiritual beliefs.

Requirements:

  • Covid Vaccination: Series required; declinations/exemptions not accepted. Proof must be uploaded.
  • Location Note: Assignment location is 2 hours from Boston, MA. Residents within 50 miles are not considered.
  • Requested Time Off: Must be noted and verified at upload. RTO requests after upload are not approved. No more than 5 days of RTO accepted.
  • Holiday Work: Must be willing to work the Labor Day holiday, including the weekend before if needed/scheduled.

Submission Requirements:

  • Experience: 1+ year current experience; must be willing to float. Previous travel and rural/small facility experience are highly preferred.
  • Certifications: BLS (AHA) required.
  • Licensing: New Hampshire state license required; Compact license allowed. NH license must be in-hand at submission.

About the Facility:

Sullivan County Health Care is owned by the citizens of Sullivan County and operated by the County Commissioners. Our facility provides residential nursing care, addressing the physical, emotional, spiritual, and psychological needs of each resident. We are Medicaid licensed for 156 beds as an Intermediate Care Facility (ICF) and certified by the New Hampshire Bureau of Health Facilities Administration. We also offer Medicare-approved Skilled Nursing Facility (SNF) services.
