Job Title: Licensed Practical Nurse (LPN) – Assisted Living

Shift:must be available every other weekend.

What You’ll Do

• Administer medications and treatments in accordance with physician orders and facility policies
• Monitor residents’ health status and communicate changes promptly to physicians and care team
• Provide direct nursing care within the assisted living setting
• Support medication management and resident safety programs
• Collaborate with STNAs/Caregivers to ensure consistent, high-quality care
• Complete accurate and timely documentation
• Assist with assessments, care planning, and resident wellness checks
• Ensure compliance with Ohio assisted living regulations and community standards

What We’re Looking For

• Active Ohio LPN license in good standing
• Experience in assisted living, long-term care, or geriatric nursing preferred
• Strong clinical judgment and attention to detail
• Compassionate, resident-centered approach to care
• Ability to work independently and as part of a supportive team
• Strong communication and organizational skills
