JOB SUMMARY
The LPN – Home Care Coordinator (Chilliwack) is responsible for delivering resident care for Independent older adults by applying the nursing process, including assessment, planning, implementation, and evaluation, in accordance with community care regulations and best practices. This is a leadership role, providing hands on health assessments, care planning, and leading home support workers in their provision of care.
QUALIFICATIONS
- Full scope LPN required
- Management experience required
- Registered with the British Columbia College of Nurses and Midwives (BCCNM)
- Minimum of 5 years experience in Community Care, Supported Living or Assisted Living (Community Care/Home Care preferred)
- Good working knowledge of the regulations established by Community Care, the Assisted Living Registrar and other regulatory bodies related to senior living
- Proficient at computer programs and advancing electronic health records
- Current First Aid and CPR certificate

- Evidence of good physical and emotional health and the capability to handle the demands of the job
- Demonstrated ability to work harmoniously to develop and foster a team approach with members of a multidisciplinary team
- Ability to respond effectively in emergency situations within BCCNM’s scope of practice
- Effective leadership skills to anticipate needs and delegate accordingly
- Ability and desire to support the organization’s philosophy, values, goals, policies, and procedures
- Ability to work effectively in an ever-changing resident-focused environment
- Comfortable making care and service plan recommendations to current/potential home care clients
- Strong administrative acumen and ability to remain organized in a complex environment
KEY RESPONSIBILITIES
LPN
- Identifies/assesses physical and recreational needs of residents upon admitting them to the Home Care Program
- Completes the necessary documentation and reporting systems to maintain accurate records and communication for the department (attendance records, assessment tools, promotional materials, calendars, etc.)
- Performs services for Independent Living Residents – administration of medication and care – ADL’S
- Initiates communication and collaborates with support services, medical professionals, and residents’ families as needed to ensure resident-focused care
- Provides wound care
- Performs quarterly assessment reviews of all residents
- Assists in enhancing the Health and Wellness program
- Attend staff meetings and educational sessions
- Ensures care staff contribute to the maintenance of resident documents and files according to established principles and charting guidelines
- Performs health and wellness assessments for residents to make care package recommendations.
- Using and managing care through the use of Electronic Health Records
- Performs other related duties as instructed
Home Care Coordinator
- Assists with the selection, hiring and orientating of care staff to meet the needs of the department
- Completes annual performance reviews and evaluations, setting goals for the year and addressing areas of performance that need to be improved for all direct reports
- Resolve conflicts arising within the department, completes investigations as part of any grievances of formal complaints
- Organizes and facilitates the immunization clinics
- Provides day-to-day support, coaching, and guidance to Care staff in provision of high-quality service
- Ensures care staff contribute to the maintenance of resident documents and files according to established principles and guidelines of charting
- Assists in developing and managing the home care budget in Garrison Crossing
- Ability to manage, support and lead the frontline care team in their use of Electronic Health Records, including training aspects
NOTES
- This is a permanent part-time position, 4 days per week
- This position will require being on call (evenings/nights that follow the dayshift worked)
- Competitive compensation based on qualifications and experience
Job Type: Part-time
Pay: $39.25-$44.41 per hour
