I. General Purpose of the Job

The purpose of this position is to supervise, provide and coordinate nursing care in compliance with facility policies and procedures, assess residents’ care needs, and take appropriate action.

Specific Duties and Responsibilities

  • Interpret existing policies and procedures to nursing assistants, restorative nursing assistants, residents, families and physicians.
  • Supervise and evaluate all direct resident care and initiate corrective action as necessary.
  • Obtain report from nurse being relieved and record sufficient information to implement appropriate follow-up action as necessary.
  • Provide report to nurse coming on duty, including sufficient information for follow-up action as necessary.
  • Take appropriate action to assure adequate staffing to meet residents’ needs.
  • Conduct resident rounds daily, reporting problems to nursing supervisor and initiating corrective action.
  • Provide supervision to Nursing Assistant providing direct resident care.
  • Follow staffing guidelines and make nursing assignments according to qualifications of staff and characteristics of the resident population.
  • Assume responsibility for compliance with federal, state and local regulations within the assigned unit.
  • Counsel Nursing Assistants and recommend disciplinary action to the Director of Nursing Service, when necessary.
  • Participate in the employee evaluation process by providing information on the performance of Nursing Assistants.
  • Administer and document direct resident care, medications and treatments per physicians’ orders and accurately record all care provided.
  • Provide instruction for appropriate nursing care to licensed nurses and Nursing Assistants.
  • Perform comprehensive assessment of residents as assigned.
  • Document comprehensive assessment of residents as assigned.
  • Develop and implement an accurate comprehensive care plan based on each resident’s needs and his/her comprehensive assessment.
  • Promptly report changes in residents’ conditions to the physician, Director of Nursing Service, and responsible party, and take follow-up action as necessary.
  • Follow facility admission procedures and communicate resident admission to other departments, physicians and ancillary service providers.
  • Competently perform basic nursing skills.
  • Follow facility discharge /transfer procedure and coordinate discharge/transfer with other departments, physicians and ancillary service providers.
  • Complete documentation related to the resident’s discharge/transfer.
  • Supervise response to resident’s call for assistance.
  • Listen to resident, family, or physician concerns and initiate corrective action and/or report concerns to Nursing Supervisor.
  • Count narcotics, check emergency kit and exchange keys.
  • Check meal trays and nourishment to ensure proper diet is served and residents are assisted as necessary.
  • Supervise the application and removal of all restraints and devices used to position and enable residents to function as independently as possible.
  • Order, receive and store medications appropriately.
  • Notify physician, when necessary, of changes in residents’ conditions, weight, medication errors, medication not available, abnormal laboratory/X-Ray results and any other unusual observations.
  • Document resident progress notes as required.
  • Document daily notes on unstable residents, Medicare residents and recently admitted residents.

General Duties and Responsibilities

  • Perform other related duties as directed by his/her supervisor in a cooperative manner.
  • Perform all duties assigned in an effective, timely and professional manner
  • Follow Residents’ Rights policies at all times.
  • Assume accountability for all data contained in the employees’ handbook.
  • Consistently work cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies and ancillary service providers.
  • Obtain continuing education required to maintain licensure.
  • Be responsible for the safety of residents under his/her supervision.
  • Come to work in a clean, neat uniform and consistently present an appropriate professional appearance.
  • Come to work as scheduled and consistently demonstrate dependability and punctuality.
  • Assume accountability for data contained in the employees’ handbook.
  • Follow established standards of nursing practices.
  • Identify safety hazards and initiate corrective action immediately.
  • Attend staff meetings, interdisciplinary team meetings, rehabilitation meetings, in-service classes and committee meetings as assigned.
  • Complete Relias trainings as assigned.

III. Qualifications

  • Must possess a valid license as a Vocational Nurse or Registered Nurse in the State of California and be I.V. certified.
  • Must have a combination of training and experience equivalent to graduation from an accredited Vocational School or School of Registered Nursing, and one year of professional experience.
  • Two years of experience as an LVN, RN, or non-professional, particularly having experience working with the elderly, is highly desirable.
  • Must be computer literate, willing, and able to learn new software as needed.
  • Must be in good physical and mental condition, capable of performing assigned tasks
    • Good health must be verified by a health screening performed by a licensed physician, including a chest X-ray or intradermal test for TB
    • Must be able to be active over 75% of the time spent on the job.
    • Expected to stand, walk, sit, stoop, kneel, and reach, while listening and talking, throughout the duration of the shift. Must be able to push/pull up to 50 lbs.
    • Must be able to hear, talk, and have a clear vision with or without corrective lenses.
    • Must be able to sit in front of the computer for extended periods of time (up to or more than 50% of the shift).
    • Must be able to use a manual pill crusher.
  • Must be able to perform in a busy environment, subject to frequent interruptions, hostile and emotionally upset residents and family members, and subject to falls, infectious diseases, substances, and odors.
  • Must be willing to work beyond normal working hours.
