Job Overview
We are seeking a dedicated and compassionate Care Navigator to join our team. The Care Navigator will play a crucial role in supporting patients and their families by guiding them through the healthcare system, ensuring they receive the necessary resources and assistance for their health needs. This position requires strong organisational skills, an understanding of healthcare processes, and a commitment to providing exceptional support to patients.

Duties

• Serve as the primary point of contact for patients, addressing their questions and concerns regarding healthcare services.
• Assist patients in navigating the healthcare system, including scheduling appointments, coordinating referrals, and accessing community resources.
• Maintain accurate records of patient interactions and ensure all information is up-to-date in the office systems.
• Collaborate with healthcare providers to facilitate communication and improve patient care.
• Educate patients about available services, treatment options, and health management strategies.
• Provide emotional support to patients and their families during challenging times.
• Participate in team meetings to discuss patient progress and share insights for improving care delivery.

Qualifications

• Previous experience in an office environment is essential; experience in a healthcare setting is preferred.
• Strong interpersonal skills with the ability to communicate effectively with diverse populations.
• Excellent organisational skills and attention to detail.
• Ability to work independently as well as part of a team.
• Proficient in using office software and electronic health record systems.
• A compassionate approach with a genuine desire to help others navigate their healthcare journey.
• Knowledge of healthcare policies and community resources is advantageous.

Pay: £11.44 per hour
