Job Overview

We are seeking a dedicated and compassionate Medical Social Worker to join our home health team and make a meaningful difference in the lives of patients and their families. In this rewarding role, you will provide compassionate support, counseling and advocacy to help patients navigate the emotional, social and financial challenges associated with illness and recovery. You will work closely with healthcare teams to ensure holistic patient care while adhering to all privacy regulations and documentation standards.

Duties

  • Conduct thorough patient assessments focusing on psychosocial needs, mental health status, and social determinants of health
  • Develop individualized care plans that incorporate counseling, resource linkage, and support services for patients and families
  • Provide clinical counseling including behavioral therapy, mental health counseling, crisis intervention, and motivational interviewing
  • Assist with discharge planning by coordinating community resources such as assisted living, hospice care, or specialized therapies like Alzheimer’s or PTSD care
  • Document all patient interactions accurately within Electronic Medical Record (EMR) systems while maintaining compliance with HIPAA regulations
  • Collaborate with interdisciplinary teams to facilitate case management, diagnostic evaluations, and treatment planning

Qualifications

  • Bachelor’s degree in Social Work (BSW) or higher; Master’s degree preferred
  • Valid state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent credential
  • Experience in home health settings
  • Excellent communication skills for intake interviews, clinical counseling sessions, and crisis management
  • Ability to handle sensitive situations with empathy while maintaining professional boundaries

Pay: $60.00 - $80.00 per hour
