!Part-Time / PRN Medical Social Worker – Hospice
Location: Houston, Texas 77054
Schedule: Part-Time/PRN
We are a small hospice agency with a low census, seeking a compassionate Medical Social Worker (MSW) to support patients and families with emotional, social, and resource needs.
Responsibilities:
- Assess patients’ and families’ psychosocial needs
- Assist with advance care planning, community resources, and crisis intervention
- Provide grief and emotional support to caregivers
- Participate in Interdisciplinary Team (IDT) meetings every 14 days
- Document visits per agency and Medicare standards
Requirements:
- Master’s Degree in Social Work (MSW) required
- Current Texas licensure (LMSW/LCSW) preferred
- Hospice or palliative care experience preferred
- Strong interpersonal and counseling skills
